A SLC truck came onto the scales overweight so driver proceeded to dump off at the dump off area. Customer returned to the scales underweight and the driver said to the scale clerk to print the ticket. Driver exited the truck to retrieve the ticket and told the clerk had just cut thumb off. I've reached out to SLC for more information or details on this accident without success.

Mostrando los 6EE was beginning to make repairs to a failed spider plate inside the log washer. EE placed left foot on the log shaft, left foot slipped, which caused left shin to make contact with the edge of the log shoe.
On February 20, the miner was being task trained to operate a haul truck. EE sat in the jump seat for approximately 2 roundtrips to the crusher. On February 26, miner informed management was experiencing lower back pain since riding in the haul truck.
The miner was bent over loading explosives into the face when a small stone dislodged striking miner on the head below the lip of hard hat, causing a small cut on head. The miner was wearing all PPE. Both miners present stated that they examined the face for loose material and found none.
Employee was picking up duo cone filters from customers floor. On standing EE felt pain in lower back.
The IE repositioned self on top of a weir which caused the employee/operator to lose visual of one another. As the 326F bucket was lowered, it brushed EE's outside right leg which caused EE's inside right leg to scrape alongside the flange of the weir. IE's inside right leg received a laceration from below the knee to the bottom of EE's calf.
