Employee was standing on the step of a bulldozer inspecting for a suspected coolant leak. EE attempted to open the front hood. The weight surprised EE and EE lost their balance falling approximately 2ft backwards to the ground.

Mostrando los 3The incident occurred at a sand and gravel mine where the terrain is primarily loose sand. The employee states EE was walking down a slope to go restart the pump that feeds water to the wash plant. The pump sets near the water approximately 15ft below natural grade. As EE walked down the slope the sand shifted under EE's foot causing EE to stumble and "roll EE's ankle".
The employee was exiting a catwalk on the shaker screen. As EE descended ladder & self closing gate began closing. EE reached forward to slow gate with left hand and was thrown off balance causing EE to grab the gate. The force of EE's weight shifting caused gate to close quickly. The employees right thumb was between the hinge & side rail. EE's thumb was pinched.
