Employee was underneath bonnet unpinning socket from cage at #4 Shaft 900Lvl landing. While employee was exiting bonnet, some build up from shaft fell through bonnet and hit employee on left eyebrow. Employee taken to ER and received 2 stitches and a prescription for an antibiotic.

Employee went to charge Milwaukee battery on charger. Charger did not function and when employee went to unplug charger EE felt a shock. Upon inspection damage was noted on the cord. Employee taken to ER for evaluation.
Employee was cleaning hardened material from around piping when released material and water caused EE to lose footing and fall into the water. Employee struck the left side of EE's body, head, & left leg/knee on piping & was briefly submerged before being assisted by a coworker. Initial treatment provided by site nurse. Employee was later removed from site pending medical release.
