Steam cleaning a radiator and steam contacted left lower leg
Employee was walking through tipple and tripped over a step. Didn't fall completely. Initial incident occurred 12/31/13, however employee didn't report incident until after Form 7000-2 was submitted on 1/10/14. Due to delay in reporting incident, reporting to MSHA was overlooked, but was reported to Worker's Compensation. An amended 7000-2 will be submitted.
Employee was exiting his truck at the scales to weigh. When he stepped down on the bottom step, it broke, throwing him backwards and he hit his head and shoulder on the platform. He passed out for about 3-4 minutes. Employee was taken to the ER by the supervisor. This resulted in a concussion and a shoulder contusion.
Employee was changing a hose on the Cat 992C Front End Loader. When finished, he raised up to come to a standing position, not realizing that the arm bracket to the mirror was above his head. He hit his head on the arm bracket, jamming his neck.
Employee was climbing into loader at beginnning of shift. Went up the steps and stood on platform. Icy conditions on platform caused employee to slip and fall. Employee fell, but caught himself with his hands and pulled himself back up.
Employee was adjusting a round hay bale as it was being put in the mulcher. Used left arm to push on bale and arm became stuck in hay bale as it was moving, pulling arm, resulting in some pulled muscles.
Employee was fueling the dozer at the end of his shift. When he was finished, he was attempting to step down from the dozer. He was holding onto the safety bars and going down backwards. When he put his feet on the ground and was still holding the bars, he heard a pop in his arm.
The coal truck driver, just finished loading his dump truck with a front end loader and was exiting the loader via the ladder when he slipped and fell onto the coal pile. He was not aware of any injury at that time. On Tuesday, January 6, 2009 he was taken to the emergency room and after evaluation and tests he underwent an emergency splenectomy.
