I.NAME WAS DRIVING BULK ANFO TRUCK FROM STRIP SHOT AREA TO LOAD TRUCK HE WAS DRIVING ONTO A STATE ROAD FROM STRIP PIT HE WAS SUPPOSED TO TURN LEFT ONTO A ROAD LEADING TO ANFO BIN BUT WENT STR AIGHT AHEAD & RAN OVER A 250FT EMBANKMENT HE EITHER JUMPED OR WAS THROWN FROM TRUCK HE WAS CRUSHED BY THE T RUCK & DIED FROM INTERNAL INJURIES (THIS IS MY SPULATION ON MY PART GIVEN
I NAME WAS WORK3NG THE MACHINE ON A BENCH GRUBBING TREES WHEN IT APPARENTLY SLID OFF THE BENCH DOWN A 35 FT EMBANKMENT AND TURNED OVER ON SIDE ACCIDENT OCCURRED WHEN TRAMMING THE MACHINE BACK TO PARKING AREA3
THE TRUCK DRIVER BACKED UP TO THE BERMED SPOIL DUMP AREA; THE GROUND BENEATH THE WEIGHT OF THE LOADED TRUCK GAVE WAY CAUSING THE TRUCK TO GO BACKWARDS OVER THE 150 FOOT HIGH SPOIL BANK AREA. THE TRUCK LANDED ON ITS LEFT SIDE. THE DRIVER RECEIVED SERIOUS HEAD INJURIES ALONG WITH OTHER INTERNAL COMPLICATIONS. THE VICTIMS DEATH OCCURRED AT 10:55 AM.
