HE WAS STANDING ON TOP OF THE CRUSHER TRYING TO DISLODGE A SCREEN WITH HIS FOOT. HE LOST HIS FOOTING AND SLIPPED. HIS FOOT CAUGHT IN A SCREEN, PREVENTING HIM FROM TUMBLING TO THE GROUND, BUT C AUSING THE LIGAMENTS AND TENDONS BEHIND HIS KNEE TO BE STRETCHED AND PULLED. HE WAS ABLE TO PULL HIMSELF UPRIGHT AND SHINNIED DOWN FROM THE CRUSHER.
EMPL WAS TIGHTENING DOWN THE SCREWJACKS THAT STABILIZE THE TRAP OF THE CRUSHER WHILEW DOING THIS EMPL STAINED HIS BACK MUSCLES NOTE SUPERVISOR WAS NOT NOTIFIED OF ACCIDENT UNTIL 8-15-88
