THE BELT ENERGIZED AROUND THE TIME EE STEPPED ON IT & THE BELT SWEPT EE OFF THE TOP OF THE ELECTROMAGNET. EE FELL APROX. 20-25 FT STRIKING HEAD & OTHER BODY PARTS.
Removing rust around the plate of the sand screw tub with grinder with 4" wire wheel attachment. Wheel caught the edge and kicked back.
The operator was putting tools in the compartment door of the crane and lost his balance and fell off the tracks
Using a 1" airgun to remove a nut from the crushing dies on the crusher. When the nut came off the bolt, the airgun kicked back and caught right hand between it and a structural part of the crusher.
Cleaning under crusher belt and a stone fell off and hit the employees back.
The operator dumped at the bottom of the stock pile all day the previous day.The next day the driver had dumped a load in the middle and admitted to overcoming the berm and the truck went over. The driver exited the truck on his own power.
