Employee was cleaning hardened material from around piping when released material and water caused EE to lose footing and fall into the water. Employee struck the left side of EE's body, head, & left leg/knee on piping & was briefly submerged before being assisted by a coworker. Initial treatment provided by site nurse. Employee was later removed from site pending medical release.

EE went on the transport bus to place EE's lunch box on a seat before the morning meeting (before start time). EE was exiting the transport bus, as EE stepped off the bus, EE stepped into a divot on the ground. EE fell to EE's hand and knees.

EE was driving EE's work truck on top of the dam, EE exited the vehicle onto the berm when the ground beneath EE gave way, causing EE to fall approximately 10 feet down the dam embankment. EE was able to climb back up the dam independently, re-enter EE's work truck, and drive to a location where Emergency Response Team (ERT) assistance could be obtained.

Emp was driving mantrip from A6 to lang shop when EE veered off the road and came into contact with rib. Emp. taken the emergency room and EE was given a prescription for Ibuprofen 800mg. No restrictions.

EE was removing an impeller from a pump. The crew attached a hanger to the impeller and EE began to backspin the shaft by pulling the belt. The momentum of the belt pulled EE's finger between the belt and the sheave. EE jerked EE's hand out, but EE's glove remained pinched. The event degloved skin and the fingernail and crushed the tip of EE's right middle finger.

EE was walking on grating on the 5th floor at the washer to retrieve a shovel while cleaning out 4/5 mudball chute. As employee was walking, EE stepped on some rocks and rolled their right ankle. EE was not carrying anything at the time. EE received prescriptions, was placed on restricted duty, and prescribed physical therapy on 2/24/26.

Employee was in the process of installing a continuous miner bit insert using a brass hammer. A piece of the insert broke off and pierced the employees hand becoming embedded. Employee taken to the ER and given two prescription medications. EE was referred to a surgeon for removal of metal.

Employee was operating a D8 dozer and started experiencing neck and shoulder pain after the dozer landed roughly from climbing over a hardpan boulder that didn't break apart when employee was placing material being dumped by haul trucks.

While tied off to angle iron, employee was climbing on top of shaker to take measurements. As employee took a step, went to grab hold of angle iron for balance. Potash buildup loosened and slipped. Employee lost balance and fell to floor. Resulted in broken left forearm.

Employee was lifting a belt clamp/valve into position then pulled to tighten on belt line, causing an injury with pain to the left lower side of back. EE was treated by a medical professional for sprain/strain, and prescribed medications/pt. Restrictions are as follows: No lifting/carrying over 20lbs, no pushing/pulling, no reaching below waist, no stooping/bending.

Employee was coming down from an 8-foot A-frame ladder when right foot slipped off the bottom step and twisted right foot causing a fracture in the 4th metatarsal bone.

Employee was unhooking a large chain being held by the crane with no load on the chain. The chain became unbalanced and run thru the hook and the end came around and struck the employee on the right flank causing a contusion and two minor abrasions. The employee was terminated for not following company policy (positive drug screen).

While EE was in the process of fueling an underground diesel mantrip, EE had stepped off of the vehicle, lost balance causing them to trip and injure right ankle. EE was taken to an offsite clinic for treatment & x-ray. The injury has been determined to be a severe sprain/strain to Right Ankle. Restrictions as follows: No walking on uneven surfaces and seated work only.

While conducting maintenance on an underground vehicle, EE was opening the lid of the ore hauler, the lid slipped and pinched their left thumb between the lid and the frame. EE suffered a partial amputation to left hand, left thumb. Restrictions are as follows: No use of left hand

EE was walking across primary flex. EE's right foot unexpectedly slipped, resulting in a "pop" sound emanating from the right groin region. Restrictions place on 1/21/2025. No bending at waist, no squatting, No lifting over 20 pounds.

The door fell on the EE's ankle and the force knocked EE to the ground. It was determined the drain valve was plugged and did not drain as expected. EE had xrays that are negative. Seated duty. Allow for sit and stand PRN for pain control allow use of non-steel toe shoes and allow for elevation of right foot on chair or can

While repairing track on continuous miner, employee's thumb was caught between wooden block and gathering pan of continuous miner. Employee was wearing gloves at time of incident. Immediate Action Taken: First aid was performed, and incident was reported to supervisor. Employee was transported to local hospital for further treatment. Investigation started.

Walking from sled truck the ground unexpectedly collapsed, EE Fell three ft, landing, right leg was positioned behind EE while EE's lt leg remained extended in the mud. MRI EE was diagnosed with Meniscus tear. Restrictions in place on 1/9/2025 no lift more than 10 pounds, twisting, climbing, squatting, kneeling, or lifting from floor to waist walking to 2 minutes every 90 minutes.

EE took toolbox out of the locker and loaded to the mantrip and felt a pop in their lower back. EE has been ordered PT 2 times a week for 4 weeks, a muscle relaxer to help with sleep. The restrictions are as follows: No lifting or carrying over 20 lbs, no pulling or pushing over 30 lbs, no reaching below waist level, and no stooping or kneeling as of 12/4/2024.

EE was walking down the steps and slipped and fell. Xray of the left foot/ankle, there is a displaced, unstable fracture of the lateral malleolus with soft tissue swelling. Further care of the fracture. Oral pain medication, and a Sugar-tong ankle splint placed on the left lower leg. No weight bearing on Lt leg

While EE was walking on the backside of the belt storage unit near 10 LML, EE walked RT foot into a unseen hole due to excessive fine dust. EE continued walking stepping with the LT Foot into another unseen hole causing the EE to feel pain in the lower back restrictions: no lifting, carrying over 20 lbs, no pushing/pulling over 20 lbs, no climbing, no stooping, no crouching.

EE was hammering when a piece of sharp steel was knocked loose and struck injured EE in the lip. EE was transported to ER via EMS EE received 6 sutures and prescribed Keflex and received a tetanus vaccine. No restrictions

Cut approximately 10 feet of cable off and went to reinstall the bucket to cable when the come along broke snatching the bucket out of EE's hand causing the left hand to slam against the handrail. Results multi-fragmented fracture to the distal tuft, IM Ancef, 9 sutures, and a tetanus shot. Restrictions No use pf LT arm

EE was involved in a motor vehicle accident side swiped by vehicle. EE was wearing seatbelt. There was no loss of consciousness. EE was ambulatory on scene. EE complains of continued discomfort and stiffness to the left hip and lower back area. An MRI was ordered and results reviewed on 11/4/24. Dr ordered PT Restrictions are no lifting or carrying greater than 20 pounds

Injured Left index finger with screw gun and became dizzy and fainted.

Employee was cleaning up some build up in between the 403 pumps, Employee slipped and fell due to the slimy conditions landing on EE's left shoulder causing loss of mobility of EE's arm. Restrictions are in place light duty no lifting/using lt arm

EE states that on 9/8/24, EE was replacing a drive belt in the tunnel in an awkward position that caused pain to EE's middle back. EE has been working without accommodations since the injury. Restrictions on 11/4 after unable to accommodate: No lifting, pushing, pulling or 10lbs of force, no climbing.

While shoveling out tail roller on Area 7 #4 Drive tail. Employee stepped back to discharge shovel full of muck back onto beltline when employee's left foot lost footing and all weight shifted to left leg. Employee felt a sharp pain down lower back on the left side and down left leg.

EE was walking going to step over a raised area right foot leading. EE stepped over in a way that when EE's right foot hit the floor with all EE's weight, EE stated that EE heard and felt a crack and pop noise from EE's knee. Majority of pain located on back side of knee, both sides. After MRI confirmation on 10/14/2024. EE has a Torn Meniscus No Restrictions.

While troubleshooting an electrical issue on the drum miner the employee inserted their hand into an electrical cabinet making contact with an energized conductor and receiving a shock. EE was given a prescription for 800 mg of ibuprofen 7-day antibiotic and Silvadene cream and antibiotic for the wounds. Restrictions - Lt Hand work only.

EE states EE closed the door to the mobile equipment onto right middle finger. EE has a distal tuft fracture in right middle finger. This is a small fracture at the tip of the finger. There is no indication for any further treatment for this type of fracture.

EE tripped on a cable hitting L shoulder, L wrist, R knee. EE did not request med attention at this time. 6/12 EE complained L shoulder still sore. 6/13/24 EE was seen and diagnosed L should sprain, contusion L wrist, R knee. Follow up 6/17. EE restriction No use of L arm.

Wound to the right elbow after striking the area against the interior of a pipe. Became recordable when RX was given and restrictions put in place on 6/6/24 Restrictions: Sedentary Duty. No use of RT elbow. RX Kelex

Operator just finished removing wet feed from discharge chute & was replacing the grating on the elevator discharge chute to close up. Operator did have gloves on & when lifted up the grating slipped out hand & ended up cutting glove & employee's finger. EE received 6 stitches & Rx antibiotic. No Restrictions. Stitches came out & had to be restitched 3/14. No use of hand till 3/27.

Employee was performing vehicle inspection on loader and was wearing gloves. During inspection was closing back door and caught finger on back gate. EE received cut on RT hand right hand ring finger. EE Has nondisplaced fracture to right hand ring finger. Restrictions are no pulling or pushing or lifting.

Abrasion on right shin after being struck by wire rope cable while replacing dragline bucket dump rope on 12/16/23. After conservative treatment, EE was given prescription medication and placed on sedentary duty on 12/26/23 resulting in the incident becoming a recordable.

Employee was moving a 12-inch by 12-inch cribbing block at #5 Shaft. While adjusting block on pallet, EE's left hand slipped off the block causing EE's ring finger to wrap around the nylon webbing handle connected to the cribbing block. Employee pulled finger from nylon webbing handle and fracturing finger on LT hand. Restrictions are: Right hand only work

In Area 22 operator was walking back from mining face when EE tripped over miner cable falling backwards. Operator stuck arm out to brace the fall causing injury to right elbow. EE was diagnosed with a hairline radial head fracture to the right arm and a dislocated right elbow. EE was given prescription narcotics for pain and general anesthesia to reduce the elbow back in place.

EE turn on pressure hose causing EE to lose footing. EE stepped on to slab base of staircase felt as EE was going to fall. EE stepped back awkwardly with LT foot causing toes to bend backward EE heard a "pop" coming from LT foot. EE was taken for X-rays on 11/6/23 EE has an "acute" fracture of the first metatarsal on left foot. EE was placed on sedentary duty.

EE was removing bolts with left hand on impact, right hand holding bolt head with wrench. Wrench slipped off pinching the tip of the rt thumb. EE received 5 sutures.

EE was kneeling underneath ladder of the dredge removing bolts on the lower flange and placed EE's fingers through the grating above EE's head to balance self. Another EE working removing the U bolt dropped bolt to the grating striking the EE hand. EE was wearing gloves at the time of the incident. X-ray done on 10/18/2023 showed nondisplaced fracture. No restrictions

Maintenance mechanic reported elbow pain due to repetitive sledgehammer use on 9/25. After conservative treatment, EE was diagnosed with tendinitis on 12/4 and prescribed physical therapy.

EE on duty using a 5-foot pry bar, left hand became wedged between the bar and a pump causing an approx. 2-inch jagged laceration to the left ring finger. Pt taken to hospital for treatment and evaluation.

EE stated the pipe pushed EE back at the level of the chest and knocked EE onto EE's bottom. The MRI was on 8/15/2023. On 8/23/23 EE was placed on restrictions.

At the time of the accident, EE was traversing through a wetland on foot while performing vegetation management practices as part of the mine's reclamation processes. As EE entered an area that was densely populated w/ various types of vegetation, a Bulrush stem slid under EE's safety glasses and made contact w/ left eye, causing an abrasion of the left cornea.

Employee was removing the lower mounting plate on the leaf spring when it was loosed it fell striking the employee's left hand ring finger causing a laceration. Employee was wearing gloves.

Moving a charging carousel for batteries, ee was using a forklift to lift a hinged structure known as a wing plate. A small piece of metal had become lodged in the wing plate's hinge. When the plate was lifted, metal was subjected to a significant amount of force which caused it to be projected toward the employee at high velocity, striking the ee in the cheek.

Employee was sitting in their vehicle during lunch while a storm was passing. Once given the "ALL CLEAR" to resume work activities, employee began to gather materials to return to work. A bolt of lightning struck very close to them. Employee fell to the ground. Foreman ran to employee's aid.

IW placed EE's hand between two sticks of rail. Eq. Operator moved the stick of rail that was being installed, without clearing red zone. Stick of rail being installed slid forward and caught the end of the index finger of the R/ hand causing laceration.

*Incident is under investigation as to whether it is work related.* Employee claims to have been bitten by a spider on right hip (under waist line of pants) that EE saw on the door while exiting the dozer cab to check the machine. Dr. diagnosis was "cellulitis". Employee did not report to supervisor until late Wednesday afternoon. Dozer is fairly new and has been maintained.
