Employee was operating a D8 dozer and started experiencing neck and shoulder pain after the dozer landed roughly from climbing over a hardpan boulder that didn't break apart when employee was placing material being dumped by haul trucks.

EE was walking across primary flex. EE's right foot unexpectedly slipped, resulting in a "pop" sound emanating from the right groin region. Restrictions place on 1/21/2025. No bending at waist, no squatting, No lifting over 20 pounds.

Walking from sled truck the ground unexpectedly collapsed, EE Fell three ft, landing, right leg was positioned behind EE while EE's lt leg remained extended in the mud. MRI EE was diagnosed with Meniscus tear. Restrictions in place on 1/9/2025 no lift more than 10 pounds, twisting, climbing, squatting, kneeling, or lifting from floor to waist walking to 2 minutes every 90 minutes.

EE was hammering when a piece of sharp steel was knocked loose and struck injured EE in the lip. EE was transported to ER via EMS EE received 6 sutures and prescribed Keflex and received a tetanus vaccine. No restrictions

Cut approximately 10 feet of cable off and went to reinstall the bucket to cable when the come along broke snatching the bucket out of EE's hand causing the left hand to slam against the handrail. Results multi-fragmented fracture to the distal tuft, IM Ancef, 9 sutures, and a tetanus shot. Restrictions No use pf LT arm

Wound to the right elbow after striking the area against the interior of a pipe. Became recordable when RX was given and restrictions put in place on 6/6/24 Restrictions: Sedentary Duty. No use of RT elbow. RX Kelex

Abrasion on right shin after being struck by wire rope cable while replacing dragline bucket dump rope on 12/16/23. After conservative treatment, EE was given prescription medication and placed on sedentary duty on 12/26/23 resulting in the incident becoming a recordable.

EE on duty using a 5-foot pry bar, left hand became wedged between the bar and a pump causing an approx. 2-inch jagged laceration to the left ring finger. Pt taken to hospital for treatment and evaluation.

Employee was sitting in their vehicle during lunch while a storm was passing. Once given the "ALL CLEAR" to resume work activities, employee began to gather materials to return to work. A bolt of lightning struck very close to them. Employee fell to the ground. Foreman ran to employee's aid.

*Incident is under investigation as to whether it is work related.* Employee claims to have been bitten by a spider on right hip (under waist line of pants) that EE saw on the door while exiting the dozer cab to check the machine. Dr. diagnosis was "cellulitis". Employee did not report to supervisor until late Wednesday afternoon. Dozer is fairly new and has been maintained.

EE was replacing a brake shoe on locomotive 3804 at South Fort Meade on the right side of journal box 2, when the shoe slipped off of the brake shoe holder pinching their right pinky between the brake rigging and a 25lb brake shoe at a drop of 3" to 4". Xray completed 4/18/22 that showed, nondisplaced fracture of the right Pinky.

Employees were connecting sections of large diameter pipe. To align the pipes, employee was using a tool known as a spud bar to line up the bolt holes while the end of a pipe was suspended from a front-end loader (forks) to aid in positioning. When the equipment operator lowered the pipe, the movement caused the spud bar to pinch the employee's finger against the pipe.

Employee stated due to operating mobile equipment suffers from right shoulder/neck pain; yesterday (10/23/2022) I noticed swelling on right clavicle area where I currently have pain for the last two months. EE saw doc and it was determined on 4/19/2023 that it was 60% work related.

EE got soda ash on hands, wiping eyes to remove sweat, soda ash got into right eye. EE was treated with prescription eyedrops.

EE was transferring from ladder to platform, slipped falling 3 feet landing on feet, felt pain in right knee instantly. BECAME REPORTABLE ON 6/23/2022 WHEN RESTRICTIONS WERE PUT IN PLACE.

The tire technician was changing a tire on a piece of equipment. While changing the tire, the air hose came loose from the tire jack and the air hose hit the employee in the head causing injury.

An employee was loading a boom lift onto the truck, while not maintaining a grip on the hand hold. The unit tipped forward and EE was ejected from the platform, resulting in a broken nose. The employee was wearing a full body harness and lanyard attached to the equipment's anchorage point.

The employee was cleaning a pedestal floor plate w/a grinder and wire wheel when foreign material got into EE's right eye. The employee was wearing Safety Glasses and a Face Shield at the time of the incident. EE immediately came down from the machine and flushed EE's eye to remove the material from EE's eye. I was notified and continued flushing the eye with eyewash.

Pressure washer wand hit index finger on right hand with pressure washer. Causing .07cm round hole in right index finger middle joint area. On May 27th employee went to hospital due to pain. EE had surgery on finger to release fluid in finger.

EE was in process of tightening the coupling bolts for the hub when EE felt sharp pain in upper left shoulder. The EE was treated by in-house ARNP. Went to dr on 5/11/2021 was given a cortisone shot and ordered PT. No work restrictions. Update: EE had surgery on 1/19/22 & returned to restricted duty 3/15/22 & full duty as of 7/29/22.

EE called said they thought they had passed out and needed medical attention. ERT Responded and took vitals which were normal. EE was taken to nurse for evaluation. Out of an abundance of caution EE was transferred to ER by Ambulance.

While climbing down off the 16 dragline EE felt discomfort in EE's left knee. EE was seen by doctor on 7/1/2021 where injury was determined and restrictions were placed on EE, making the injury recordable.

Employee was in the process of moving a portable water cannon while performing housekeeping duties. During this process, they pinched a thumb between the handle of the water cannon and a pipe resulting in a laceration to the right thumb.

As an impeller was lowered, it made contact with the side of the shell causing a metal particle to become dislodged and enter miner's eye. Approved safety glasses were being worn at the time. Eye doctor had to physically remove debris from eye and miner was issued prescription eye drops.

Contractor's EE was struck in the back of the head and neck by a clevis pin that came loose from the clevis and factory tow hook on a slider bucket truck. The pin penetrated through the D8 steel guard and window. The stuck equipment is owned and operated by another contractor of Mosaic.

Employee climbed up on top of a 48" pipe to verify that the valves had closed, while employee came down, a foot slipped and they caught themselves with right hand and felt pain in right shoulder. Results of additional testing conducted on October 29, 2020 identified a tear to the rotator cuff that will require surgery.

When dismounting equipment (tractor) employee felt EE's right knee pop. Miner was treated conservatively, after worsening pain the injured was placed on restrictions by physician until diagnosis from orthopedic physician.

While lining up pipe to bolt flange the pipe sprung back and hit miner in the knee. On 5/21/2020 miner had x-ray which was negative. After continual discomfort miner had a MRI on 6/29/2020. MRI results on 7/1 revealed a fracture to the end of the fibula.

Miner experienced pain in neck, shoulder & arm while operating bulldozer. On 7/27/2020 Neurologist deemed illness (pain/inflammation) was work related and prescribed physical therapy.

Miner was loosening a king nipple fitting from another pipe assembly. While pulling pipe wrench, the jaws on the wrench slipped and as the Miner reached to catch the wrench EE bent finger. Injured received an injection for trigger finger on 6/11/2020.

On 12/5/19 employee experienced back pain while pulling & lifting dragline power cable onto the tub. Employee was treated conservatively; Rx meds were prescribed and MRI scheduled on 1/24/19. Restrictions were issued on 1/28/20.

Employee was exiting a boom truck when foot missed the bottom step resulting in a backwards fall of 30 inches. The EE broke fall by extending left arm behind self, resulting in a fracture of the left wrist.

EE was installing new posts for sign that had been run over. EE had stepped on rear bumper to get to drill in bed of truck. EE slipped off bumper and struck fore arm on sign post causing laceration. EE received 13 stitches in the fatty area of arm. No med. prescribed

EE was using John Deere Farm Tractor to pull old hoist cables out of dragline mining area to be cut into drag cables at a later date. While exiting the John Deere EE stepped down to what appeared to be level ground and rolled EE's ankle and heard a pop. Not a reportable until 6/7/2019 when Dr. prescribed RX

Fieldman Leaderman was stepping up onto the back of a float truck and felt knee pop. Injury was not to be determined to be reportable until 4/30/2019 when employee was prescribed Physical Therapy.

Electrician was installing a label on the 11 dragline #5 matrix lift pump substation and stepped in the sled pull point opening with left foot. The employee fell backward resulting in a fracture to left leg.

EE was pulling on dragline trail cable and stepped in a rut and twisted EE's knee was checked out ERT member and taken to the clinic nurse checked EE out nurse will be checking in on EE and EE can return to work EE's next day of work. EE continued to have pain so EE was sent to DR and was prescribed physical therapy on 4/4/2019

EE was assisting in the replacement of the #3-16 impeller. As the EE was removing the last nut on the faceplate, the faceplate shifted forward and pinched the EE's hand. The Nurse Practitioner was consulted and due to the nature of the injuries, 911 was contacted to transport the EE to the hospital for further treatment.

1) Operator distracted by cell phone. 2) Operator should not have been in the seat while greasing was ongoing. 3) Operator failing to notify anyone that operator was moving equipment.

EE reported injury to supv on 10/11/17 and stated it happened about a mo prior while EE was wk on an overhead pipe EE felt a twinge in left shoulder. EE was sent to nurse who ordered an MRI. Results came back on 11/16/17 and showed a focal partial tear of the supraspinatus tendon and a partial tear of the bicep of the left arm. EE was placed on restricted duty at that time.

EE states flipped a tractor cable spool over to put cable on tractor and felt pain in left neck and shoulder

EE was diagnosed with ulnar neuropathy in left hand based on years of repetitive use, and was issued a prescription on 9/14/17 for physical therapy with no work restrictions. Due to communication outages caused by Hurricane Irma, medical staff was unable to provide documentation until 9/22/17.

EE was getting out of work truck when a Black Widow spider bit EE on the left hand finger

EE was attempting to break bolt free by pulling down on wrench, felt pop and pain in left shoulder. EE saw co nurse and was given OTC meds. On 12/19 EE returned to see company nurse and was given scrip for an MRI on 12/23. Based on MRI results on 12/27 EE was put on work restriction.

Mechanic was walking away to secure a safe distance in between themselves and the discharge line, the discharge line slipped off of the flange and shook the line causing the debris that was lodged in line to blow out. EE attempted to run from the blowing debris and in the process stepped on a large rock and rolled left ankle.

MS FOREMAN AND MS JOURNEYMAN WERE REPLACING PIPING `T's ON THE SIZER IN THE WASH PLANT. THEY SUCCESSFULLY DISASSEMBLED (2) PIPE `T'S. WHILE DISASSEMBLING THE 3RD `T', A COMPRESSION PIPE CONNECTION FAILED. THIS CAUSED THE PIPE AND `T' TO FALL APPROXIMATELY (30'), STRIKING THE JOURNEYMAN ON TOP OF THE LEFT FOOT ABOVE THE STEEL TOE.

EE walked in between rock box and flex. EE signaled, crane operator understood to be "Boom Up" suction pipe, so final tie in could be made. After signaling crane, EE moved into the line of fire. When crane lifted suction, stored energy caused flex to swing back into the rock box. EE who signaled the crane operator was briefly caught between flex and rock box.

Employee encountered pain in right shoulder while in process of servicing pump rockbox door 9/24/15. EE was treated conservatively at the time. On 2/9/16 employee went back to clinic, he was re-evaluated and had an MRI On 3/4/16 MRI results determined that he has torn the ligament on his right shoulder.

EE was walking down a slope to a flange that needed to be cut when his right foot got stuck in the mud and he twisted his knee. Knee continued to bother employee: EE was later sent for MRI which revealed a torn lateral meniscus and became recordable 9/1/15.

The employee seen the nurse at the dispensary for numbness in his right elbow and right wrist. Employee was diagnosed by Doctor as having carpal tunnel that requires surgery.

While hauling fill, the employee hit a series of holes in the haul road causing injury to his back

Mechanic was installing tail bearing housing while holding housing with one hand, mechanic was using a hammer to help drive the housing on when he struck his ring finger on the left hand hitting his finger nail. Mechanic was wearing all proper PPE(work gloves) at that time.

The float crew was unbolting 20" pipe near the #11 Pit. The leaderman was walking along side of the pipeline toward the next flange and stepped on soft ground, fell, twisting his Right knee.

Employee was driving locomotive down the main line to retrieve an empty rail car set. The weather conditions were foggy and the locomotive collided with loaded Big Bend cars that occupied the same track. Employee suffered a 2 inch cut on his forehead; he also identified his neck, knee, and elbow as hurting.

Employee was bolting up a 24" pipe in north bowlegs area, as he went to carry the impact gun to the flanges stepped into a wet area and his left foot went down about 8" to 10" into the mud. He went down at an angle and he felt a tingle in the top of his left foot as he fell to his right knee it twisted and his leg was bent underneath him and he felt pain in his right knee.

Plant maintenance mechanics were replacing an old 30 ft piece of suction pipe. When they picked the pipe up the corroded end broke off, the end of the pipe kicked up and hit one of the maintenance mechanics in the face/neck area causing a laceration.

Employee was slipping a cable choker, which was attached to the John Deere tractor, through the dump rope bucket eye when the choker tightened unexpectedly. This motion resulted in the pinching of the right index finger between the choker loop and the eye of the dump rope.

2 EE replaced broken spider 605 log. When completed, inspected tail bearing of log shaft. Bearing worn on bottom; rotated to top side. To rotate it 180 degrees, shaft lifted to replace bearing housing. Come-along used to lift shaft. 1 employee lifted bearing housing put it on shaft, aligning four bolts. Log off center lifted repeatedly. Part weighs 100 pounds. EE felt pain in back.

Employee was reinstalling a water spray header on 601 log after installing new log shafts. Employee was attempting to align the spray bar header assembly using his arms and body to position the assembly. After re-positioning his body, the employee felt a pain in his left lower rib area.

Employee was returning to his truck after checking #5 water return truck, he stepped over a power cable between the pump sled and the switch gear sled. While doing this he stepped in a hole and fell forward, bending his left knee backward.

There was not an accident on the mine site. The incident was attributed to an individual's pre-existing personal health condition. An employee operating a crane was observed as non responsive. Our Emergency Response Plan was initiated. 911 was called. EMS arrived on the scene. The employee expired at a local hospital later the same day.

EE finished cleaning second floor offices. EE started down stairs leading to first floor slipped and fell landing on backside.

Employee was measuring a gap in the pipe line at the mud ball pit. He squatted down to look back at the flex and when he stood up he felt a sting in his right knee. He did not report this to the supervisor, the next day the employee was getting into the truck to go to his job site and felt the sting again. At 12:00(noon) the employee could not put any weight on that leg.

Employee was swapping a motor and pump from one base/sled to a new base/sled, the employee stepped across the old base/sled to the new base/sled, when stepping onto the new base/sled he turned wrong and pulled right calf muscle.

Float Crew Employee attempted to cross over matrix line to inspect pump for leak. Employee stated he sat on pipe and threw both legs over the pipeline. While attempting to stand back up, his right foot slipped and turned over sideways.

Employee was moving an old toilet that was on the ground beside a dumpster, the tank busted and cut his finger through his glove.

Two mechanics were installing a resistor bank on dragline 16. They brought it in to the machine house with the side winch and set it on the floor. They manually moved it across the floor to position it and pick it up with the main hoist. Employee caught his pinky finger on his left hand between the framework and a lube tank, cutting it at the first joint.

While stepping into fuel containment area IW slipped and fell causing injury.

The injured employee had an open end wrench in his left hand. He slipped while ascending a pile of fractured surface clay. In an attempt to prevent himself from falling, the injured employee extended his left hand to catch himself. The employees thumb was caught between the wrench and the pipe. The force of impact resulted in a fracture of the thumb.

Employee was stacking wood cribbing weighing 30-35 lbs onto a forklift and felt a pain in his lower back.

Employee was on the SFM 1 clay settling area checking water level at the #12 Spillway. After dropping in 2 boards at the spillway, the employee was climbing back up the stairs when he noticed wasps around his legs and some flying towards his face. Employee swatted at wasps rapidly when he felt his Left shoulder "pop". In addition, he was stung by a wasp on his Left hand.

Employee was welding/gouging under a dragline tub when a piece of metal particle entered his left eye.

Mechanic was removing bolts from a hammer mill door utilizing a 3/4" impact wrench fitted with a 3/4" to 1/2" adapter to accept the smaller socket. The adapter reportedly failed causing the socket to spin off the bolt head, striking the employee in the mouth.

Float Plant Assistant was replacing a 100 watt florescent light bulb above the burn out station. As the employee went to remove the light cover, he bumped an unsecured Plexiglas plate with his right arm. Once he removed his right arm, the Plexiglas slid down and struck the employee's left hand right above his wrist. The employee's left hand immediately began to swell.

Replacing 3-ways on dump chains and the bridle pins (1 3/4" x 9") He picked the bridle pin (with the top "keeper" in place) up with his right hand, by the top(above the "keeper") He started to drop it into the hole, his left hand was next to the hole, the bridle pin slipped out of his hand and left thumb was caught between the pin keeper and the clevis.

replacing return rollers on the pebble stacker conveyor. He was on the south side about a 1/4 of the way up. Employee was lying on right side of grading, facing the conveyor with his head towards the top of incline. He was swinging a 4 pound hammer to knock the rollers out. He felt a sharp pain in his right shoulder. He notified his supervisor and finished the day up.

Employee was taking pin out of clevis that was located on the face plate. It had a bar in the hole to loosen it and the bar slipped out of the hole and mashed his finger on the shell.

Employee was exiting the restroom across the hall from the West training room when the door closed on his right foot, twisted his knee and he fell to the floor.

While driving on bpm berm; IW was distracted. Upon, his looking back to the road; he was in wind row pulling him over the edge of the berm.

Field man was assisting in running matrix line with leader man. While working, he bent down(squatted) to see pipe alignment and felt sharp pain in right knee. Employee did NOT report the injury when it occurred and continued until shift was completed.

EE was welding and grinding in right side of ballest. Unknown how or when foreign body entered eye. EE was wearing PPE.

As part of 16 dragline repair day employee was inserting a 6 diameter x 18 long square headed pin into the hitch plate on the bucket. EE rotated the pin to line it up with the hole and once it aligned the pin rapidly fell through pinching the EE's pinky finger between the head of the pin and the hitch plate. EE was wearing gloves at the time of the incident

EE was relocating fan, stuck fingers through the guard and fan blades struck fingers.

Cleaning oil spill (which ocurred moments prior during repair of oil line on cab side of 10 dragline placing crocker sacks & soak pads on ground) employee slipped & fell landing on back. EE had placed soak pads on spill stepped onto pad which slipped causing EE to fall forward. Returning to feet, EE attempted to walk out of spill and fell for second time injuring his back

Employee was at the 12 pit preparing to drill out broken stud on stuffing box when the drill mechanism that powers the drill was inadvertently contacted causing the drill bit to contact his left thumb resulting in an avulsion/laceration.

EE was walking thought tall growth using machete. Machete slipped out of hand and cut left shin.

July 28, 12:15pm employees at 10 dragline socketing dump rope. Oiler using pry bar to set rope in socket groove. Bar slipped, oiler fell forward & tripped over drag socket. Dragline operator reached to catch oiler; oiler's boot contacted operator above right ankle. Operator reported 7/31 at 4pm. Mosaic denies this event constitutes a recordable work-related injury under Part 50.

Right middle finger caught between hammer handle and brass ring causing a laceration.

Drag system kicked out on overcurrent. Electrician was called to machine and reset. Within 5 minutes drag system tripped out again. Electrician checked back of machine and reset once again. Within a few minutes, it kicked out a third time. Electrician went to back of machine to check the drag motors and noticed fire coming from compartment underneath #1 Drag motor.

EMPLOYEE WAS TURNING 20" 90 ELL IN CYCLONE RETURN WATER LINE, EMPLOYEE BUMPED HIS RIGHT ELBOW ON A BEAM AS HE WAS TIGHTENING THE BOLTS IN THE 90 ELL. TREATED CONSERVATIVELY, SEEN BY ORTHO FOR CONTINUED PROBLEMS WITH RIGHT ELBOW. REVIEWED MRI AND ADVISED INJECTION UNDER FLUORSCOPY TO SEE IF PAIN DECREASES WITH MEDS.

Employee was getting back on tractor when he slipped and fell app. 6-7 feet off the tractor to the ground landing on his buttocks. Originally reported as minor first aid on 11/22/2008. After Medical evaluation on 12/9 it was determined that he needed further treatment.

Employee was using a pry bar to raise coupling on lighting gearbox. The pry bar slipped, pinching left pinky finger between pry bar and edge of gearbox. Incident was originally reported on 11/19/2008 as a minor first aid laceration. Became recordable after x-rays on 11/26 revealed fractured tuft of L pinky finger.

Foreign body went under face shield and got into right eye.

Employee was closing a transformer pole top type switch. When Employee closed the switch he hit a piece of metal angle iron with his back on the left side just above his kidney.

On 9/9 employee was using a power cable hotstick to lift cable off the ground and felt sudden pain in back and heard a popping noise. Physician treated conservatively until 9/13, when he was placed on sedentary restrictions, due to continued symptoms of back pain.

Employee was welding on the bucket arch. He had his welding hood, safety glasses, and a sweat band around his neck. He stopped for a short break and to wipe the sweat from his face, when a piece of metal went into his eye.

Dragline Operator was struck on leg by a board thrown to him by another employee. X-ray following day was negative. On 07/23 employee called to report increased pain and swelling of injured leg. Antibiotics and Doppler exam ordered. Became recordable when meds were prescribed(7-23).

The EE was carrying the LOTO Lockbox and was unable to access control sled. EE set the Lockbox on the ground and walked over to discuss with the Float crew in their truck. After completion of discussion, EE turned and stumbled over the LOTO Lockbox. EE fell to ground and landed on shoulder causing damage.

Notified from Audiologist on 5/29/08 of recordable hearing loss.

Employee was climbing steps. Tripped and fell forward, and caught himself with left wrist. Wrist hyperextended. X ray revealed a complete fracture.

Employee was working on removing pin on the boom intermediate cable.

While hooking up transport, trailor moved forward and rolled over employees foot.

Fuel truck driver stepped down out of the cab and twisted his ankle on a tire rut.

Employee was stepping off of forklift.

Employee was hooking up a wire rope to the rear of the dozer when the shackle slipped out of his hand and struck a finger on his left hand, causing a cut and bruise. He received a few stitches to the wound.

Employee slipped off ladder and landed on metal sled on right elbow. Treated as first aid, physician placed on restricted duty 2/12/08.

Dragline Operator was arriving for work (5:45 AM)and walking from his personal vehicle to the #11 Dragline. The dragline had swung around to a position that would allow the operators to mount and dismount the machine thus giving them lighting to and from their personal vehicles. The EE walked about 100' and his foot stepped into an uneven tire track and his right ankle turned.

Employee was helping a mechanic to replace a hydraulic hose on the pan. He was applying force with a wrench when it slipped. He cut the palm of his left hand.

Upon getting into the cab of the backhoe; operator was stung by a wasp.

Dragline Operator was stepping down off the stairs to the ground below (approx. 24"). When pressure was applied to the lower foot, the ankle twisted on the ground below. EE felt sudden pain in ankle and sat back on lower step. Pain was severe and EE laid back on stair and lost consciousness.

While cleaning in dragline around swing shafts, employee turned backwards, raised leg and knee hit a metal electrical box, causing a small laceration and contusion to left knee. Treated as first aid until June 29, 2007 when physical therapy was prescribed.

Employee walked out to go to Rent-a-Jon across the road at the pit. He was returning to pit car, & as he was about to step onto the stair, the ground gave way underneath the car and he fell forward. He caught himself from going to the ground on the middle handrail and immediately felt a sharp pain in the shoulder. On 6/5/07 prescription medication was ordered.

EE sustained noise-induced hearing loss.

EE was working in close proximity to a suspended tunnel gate during installation. The gate was suspended utilizing two come-alongs, one chain fall, and two bolts. One of the bolts loosened resulting in a load shift. This caused the gate to rotate and reposition impacting the EE in the forehead.

EE was removing bolts from the end cap of the #12 DL Hoist Final Drive Pinion Assembly. While attempting to remove the lower bolt with a half inch impact and socket, the socket became unseated from the bolt head creating a pinch between the end cap and socket. His thumb was caught in between the socket and the end cap causing the fracture to the left thumb.

Pulled muscle while floating screen across ditch.

After installing and fastening bolts on guard (extending hands over head) employee twisted right knee. On 4/2/07 employee was walking in washer and knee gave out. It was determined the incident was result of 3/20 incident and employee was given pain medication and restrictions.

On 2/22/07 employee complained of numbness & tingling in both wrists. Treated by in house PA and referral was ordered - No determination made as to work related incident. On 6/12/07 referred for 2nd opinion and was determined to be work related (carpel tunnel syndrome - both wrists).

An employee was lifting an 80 lb. part off the truck and placing it on the shoe of the dragline. When the employee went to set the part down his hand was in a pinch point resulting in a fracture to the tip of his Rt. Thumb.

Pulled muscle in L side/hip while lifting a pump to set.

Employee was tightening bolts, using a hammer wrench and 10# sledge hammer. As he went to swing sledge to contact hammer wrench, sledge hit support metal and glanced off, hitting left thumb. Employee was wearing gloves. He was given restrictions due to fracture to distal tip of thumb.

Employee was assigned to install mailboxes on aluminum tubing. Immediately after engaging the saw to cut a notch the saw jumped/kicked back & contacted his hand causing laceration. Employee was not wearing gloves.

Employee was lifting baskets with tools, rigging equipment, etc. on 8/24 and felt pain in back but returned to normal duties. On 9/30 the employee was descending steps and foot slipped causing both feet to come out from under him; he sat down on step after landing. On 10/2 employee reported lower back pain. Physician ordered MRI and on 10/19 incident was determined work-related.

Handrail came away from the D/L wall and pulled toward employee, causing hand to pull across a broken jagged edge and cut his hand. Handrail had broken only within the past couple of hours and had not been recognized.

Mashed finger in toolbox.

EE was using an impact wrench when the air hose broke and slapped his right knee. Later in the day, he stepped out of the truck and twisted the same knee. After continued complaints, he was seen by Orthopedic surgeon. He was placed on light duty and given prescription on 4/24/06.

EE was attempting to mount the dragline. He had tool belt on left shoulder and began to pull up with both hands. Left hand slipped and EE continued to pull self up on board with right arm. Felt a stinging pain in R shoulder continued to work. Notified supervisor later that evening. on 1/27/06 EE had additional complaints Evaluation made by Dr. MRI revealed bicep tear.

Rainy, wet weather & not keeping 3 pt. contact while dismounting caused slip on muddy rails.

Employee was washing up at south end of washer and felt a sting on right leg. He thought nothing of the bite and continued working remainder of shift. Reported discomfort on next shift and received first aid. On 1/16 ee was prescribed antibiotic and placed on restricted duty.

EE WAS IN PROCESS OF INVESTIGATING AN OVERHEAT CONDITION OF A VEHICLE. AS HE WAS REACHING IN THE GENERAL DIRECTION OF THE RADIATOR CAP, CAP BLEW OFF AND SPRAYED UPPER PORTION OF BODY WITH STEAM, RESULTING IN 1ST AND 2ND DEGREE BURNS TO RIGHT FOREARM AND WRIST.

MECHANIC CAUGHT HIS LEFT FINGER BETWEEN TWO FLANGES. (EMPLOYEE STATED THAT INSTEAD OF SETTING HOSE DOWN, HE JUST TOSSED IT DOWN, NOT THINKING OF WHERE HIS HAND WAS). HE SUFFERED AN AVULSION TO THE DISTAL TIP OF THE LEFT RING FINGER REQUIRING 7-8 SUTURES. EMPLOYEE RETURNED TO RESTRICTED DUTY.

LOADER EE WAS OPERATING FOR CLEANUP AROUND MUDBALL REJECT PUMP TANK BECAMSE STUCK & WHEN IT WAS PULLED OUT THE FRONT TIRES DROPPED INTO RUTS MADE BY REAR TIRES, JARRING OPREATOR INSIDE THE CAB. EE PLACED ON RESTRICTED DUTY 4/29/04

Employee was standing on the tail pulley guard approx. 3' above the ground when the chain on the valve slipped causing employee to lose footing and fall to the ground(EE was exerting too much force on the come-a-long.)

EE WAS ATTEMPTING TO CLOSE LOWER DOOR OF RAIL CAR WITH PRY BAR. THE PRY BAR SLIPPED & HIT EE IN FACE CAUSING A LACERATION TO RT. UPPER LIP. 2 SUTURES WERE REQUIRED TO CLOSE THE WOUND.

WHILE IN PROCESS OF REPLACING IMPELLER ON A CONDITIONER THE EE REACHED DOWN TO RAISE IMPELLER OUT OF FEED TO INSTALL A CABLE SLING. EDGE OF IMPELLER CUT THROUGH HIS GLOVE & INTO THE FINGER. EE WAS WEARING WORK GLOVES & ALL REQUIRED PPE. LACK OF SKILL-INADEQUATE PRACTICE. VERY FEW STAINLESS STEEL IMPELLERS ARE USED IN THIS SERVICE. EE DID NOT RECOGNIZE POSSIBILITY OF EDGE BEING AS S

EE WAS WORKING ON ELECTRICAL STARTER SLED IN SHOP FOR RECONDITIONING AND REPAIRS TO THE HIGH VOLTAGE DISCONNECTS INSIDE CABINET. A 4'LADDER WAS LEANED AGAINST CABINET IN THE FOLDED POSITION & EE WAS STANDING ON 1ST RUNG. THE LADDER COULD NOT BE PROPERLY OPENED SINCE SLED BASE WAS NOT WIDE ENOUGH. LADDER SLIPPED CAUSING EE'S LT. ARM TO COME IN CONTACT WITH BRACKET. CUT UPPER ARM.

EE STRUCK HIS HEAD ON A PIECE OF ANGLE IRON WHICH WAS WELDED ON BOTTOM OF DRAGLINE HOUSING. BRACKET USED TO HOLD AN OLD TELEPHONE BOX FOR GROUND CAB COMMUNICATION, THAT HAD BEEN REMOVED. THE TWO ANGLE IRON BRACES WERE LEFT EXTENDING APPROX. 1'EACH. THIS IS AN AREA WHERE HARD HATS SHOULD ALWAYS BE WORN.

EE WAS WORKING IN AN AWKWARD POSITION & TWISTED HIS BACK

EMPLOYEE WAS ATTEMPTING TO SHORTEN A PLASTIC DRAIN LINE WITH A POCKET KNIFE. SHE WAS HOLDING THE TUBING IN A "U" POSITION AND ATTEMPTED TO PUSH THE KNIFE AWAY FROM HER TO COMPLETE A CUT. HER L EFT INDEX FINGER WAS STABLIZING TUBE AND KNIFE CUT THROUGH TUBING INTO TOP OF FINGER. LACERATION REQUIRING THREE SUTURES.

AS EE STEPPED OFF LAST STEP, FOOT WAS PLACED IN AN AWKWARD POSITION. CONTINUED PAIN AND SWELLING RESULTED IN EE BEING PLACED ON RESTRICTED DUTY,01/14/03.

EE WAS CHANGING AN OUTSIDE LIGHT AT PIT CAR AND STEPPED ONTO A 20' WATER LINE TO CROSS TO THE OTHER SIDE. FOOT SLIPPED OFF PIPE. AREA AROUND THE PIT IS TYPICALLY WET AND CAUTION SHOULD BE USED WHENEVER AN EE HAS TO CROSS OVER ANY PIPE LINE. PROPER FOOTING AND SPEED SHOULD ALWAYS BE USED WHEN PERFORMING THIS TASK.

WRENCH HAD BEEN LAID UP ON ANGLE IRON WHILE MECHANICS INSTALLED THE BOLTS. NORMAL VIBRATION APPEARS TO HAVE BEEN ENOUGH TO SLOWLY CAUSE WRENCH TO FALL APPROX. 2'.

EMPLOYEE WAS OPERATING FRONT END LOADER AND WAS THROWN AGAINST THE FRONT WINDOW GLASS WHEN THE FORKS OF THE LOADER HIT THE GROUND AND DUG IN CAUSING THE LOADER TO ABRUPTLY STOP. EMPLOYEE WAS K NOCKED UNCONSCIOUS AS A RESULT OF THE BLOW TO THE HEAD. EE WAS WEARING SEAT BELT.

EE WAS WALKING DOWN STAIRS (WITH WET BOOTS) AND FOOT SLIPPED OFF LAST STEP, TWISTING ANKLE.

EMPLOYEE WAS HOOKING A CABLE FROM ON D9 TRACTOR TO THE BLADE OF ANOTHER D9 TRACTOR. AS THE SLACK WAS BEING PULLED OUT OF THE CABLE, THE HOOK ROTATED PINCHING THE RIGHT THUMB BETWEEN THE HOOK A ND TRACTOR BLADE. THUMB WAS FRACTURED AND EMPLOYEE PLACED ON RESTRICTED DUTY.

AS EE WAS DISMOUNTING TRACTOR, HE FELL AND HIT HIS CHEST ON THE TRACK. IT WAS RAINING AND THE TRACK WAS SLICK. EE LOST CONSCIOUSNESS. HE WAS PLACED ON RESTRICTED DUTY.

NOT REALLY TOO SURE, BUT OPERATOR OF HEAVY EQUIP. MAY HAVE BUMPED HIM TOO HARD BUT THINKS HE HIT A BUMP IN HAUL ROAD TOO HARD, THEN WAS AGGRAVATED BY CONTACT WITH EQUIPMENT.

MECHANIC WAS WALKING THROUGH HALLWAY TO ELECTRICAL SHOP. HE STUMBLED, TRIED TO CATCH HIMSELF, AND STEPPED ON BOARD BOLTED TO THE FLOOR. THIS RESULTED IN HIM TWISTING ANKLE/FOOT.

EE & TWO OTHER MECHANICS WERE REMOVING OLD PIPE LINE FROM WASHER. ONE WAS OPERATING CRANE & ONE WAS FLAGGING. THIS EE WAS HOLDING ONTO PIPE UNTIL IT WAS CLEAR OF PIPE RACK. AS EE WAS GUILDING THE PIECE BEING MOVED, HE FELT THE BURNING SENSATION FROM OVER-EXERTION.

EMPLOYEE WAS HANGING IMPACT RELEASE WHEN ICIDENT OCCURRED. EMPLOYEE HAD LEFT HAND ON FLANGE OF BOTTOM PIPE WHILE PULLING DOWN ON IMPACT RELEASE.

EMPLOYEE WAS ASCENDING UPWARD IN A NEAR CRAWLING POSITION. WHEN HE ATTEMPTED TO COME TO AN UPRIGHT STANDING POSITION, HE STRUCK HIS HEAD ON THE JIB OF THE CRANE, ALLEGING MOMENTARY LOSS OF CON SCIOUSNESS.

MECHANIC WAS PULLING LOG SHAFT UP HILL WITH 3/4 TON COMEALONG. THE HOOK ATTACKED TO COME-A-LONG HOUSING BROKE CAUSING MECHANIC TO FALL BACKWARDSONTO LOG SHAFT CAUSING CONTUSION TO LOWER BACK A ND GLUTEAL AREA.

EE WAS INSTALLING 8" PIPE LATERAL ON COARSE FEEDTANK & REMOVING OLD PIPING. HE WAS LIFTING A SHORT SPOOL BY HAND. THE SPOOL DROPPED DOWN, CATCHING HIS LEFT INDEX FINGER BETWEEN THE FLANGES OF THE OLD SPOOL CAUSING A LACERATION THAT REQUIRED4 SUTURES TO CLOSE WOUND. EMPLOYEE WAS WEARING GLOVES.

AFTER ADJUSTING A BEARING UNIT ON PUMPING SYSTEMSEALING WATER PUMP EE WAS DISMOUNTING THE FLOAT WHICH WAS ABOUT 28" FROM SLED TO GROUND. HE WAS HOLDING A 3/4" COMBO WRENCH & CRESCENT WRENCH IN HIS RIGHT HAND, HOLDING ONTO THE HANDRAIL WITH HIS LEFT HAND. WHILE BACKING OFF THE PLATFORM HESTEPPED ON A 2 1/2" ANGLE IRON BRACE & TRIED TO CATCH HIMSELF WITH HIS HAND & HURT HIS SHOULDER.

EMPLOYEE WAS REMOVING BOLTS FROM A FLANGE ON THEHYDRAULIC LINE WHEN HE FELT A PAIN IN LOWER BACK

EE WAS DISCONNECTIN TWO FLEXIBLES TO MOVE TO A WATERJACK AND WHEN THE LAST BOLT WAS TAKEN OUT, IT MOVE A COUPLE OF FEET THUS HITTING EE IN THE HEAD. THE FLEXIBLE HAD STORE ENERGY WHICH THE CRE W THOUGH HAD BEEN REMOVED WHEN THE TRACTOR OPERATOR STRAIGHTENED OUT THE FLEXIBLES.

EE WAS OPERATIN A TRACTOR AND CONTINUOUSLY TWISTING AND TURNING TO LOOK OVER SHOULDER TO OBSERVE THE OPERATION OF THE WINCH. AT END OF SHIFT HE COMPLAINED OF LOW BACK PAIN. HE WAS SEEN BY THE PHYSICIAN AND PLACED ON RESTRICTED DUTY FOR TWO DAYS.

IMPROPER BODY MECHANIS & LIFTING TECHNIQUES

EE HAD JUST FINISHED OPERATING THE CRANE AND WAS PARKING THE PIECE OF EQUIPMENT BEHIND THE MAINTENANCE SHOP. WHEN HE EXITED THE CRANE HE MISSED THE LAST STEP, FELL BACKWARDS, STRIKING THE BACK OF HIS HEAD AND LEFT ELBOW ON THE ASPHALT.
