Weather was clear. Climbing onto D8 Dozer, twisted left knee. It was sore and was swollen in the evening.
Foreman was replacing clips on the conveyor belt while the person of interest was handing the foreman parts standing on the ground when EE got light headed, and sat down on a cement block. Foreman asked if EE was Okay and EE said no, and then EE bent over and started falling forward. The foreman grabbed EE and called for help, called 911, and two other employees performed CPR.
Screen belt bearings went out. Shut down crusher. Cleaning return rollers on belt with hammer. Under screen belt caught hammer and pulled hammer and Rt arm into the belt. Laceration on Rt hand, friction burn on forearm.
Employee went to step up on the cement block and knee made a pop sound. Employee was limping after that point.
EE was finishing the mechanical repair to a D8 Dozer (tighten fan belt) when his pry bar slipped and he lost his balance. This caused him to fall off the track of the dozer. Upon hitting the ground his left leg broke just below the knee.
