EE positioned extension ladder against Eagle sand screw. EE mentioned he checked the positioning of the ladder twice. The ladder lost footing on one side and slid along the screw causing ee to fall and impact the sand below. Hoses are provided to grease the 2 bearings on top of the screw that are reached from ground level.

Mostrando los 5An EE gave IE a pocket knife at their lunch. EE lobbed knife from his loader to IE, who was in his truck. IE reported to owner that he cut his hand when he caught the knife, which must have been opened when tossed back to him. IE went to ER and there was possible tendon damage and ER temporary closed wound. IE had surgery on 9/5/11 to reattach the tendon and repair nerve damage.
EE FELL APPROX 5' FROM LADDER WHEN LADDER WENT OUT FROM UNDERNEATH HIM. EE HAD LADDER AT TOO SHALLOW OF ANGLE AGAINST WALL-DID NOT WAIT FOR ASSISTANCE TO HOLD LADDER-NYLON PADS ON LADDER SLID ON CEMENT FLOOR. XRAYS WERE TAKEN, NO INJURIES REPORTED.
SHAFT ON FEEDER BELT SHIFTED TO ONE SIDE. EE WAS ATTEMPTING TO MOVE SHAFT BACK WITH 2 MALL SLEDGE HAMMERS. RIGHT THUMB GOT BETWEEN HAMMERS. REQUIRED 7 STITCHES NO LOSS WORK HOURS.
USING POCKETKNIFE TO CUT PC OF PLASTIC AIRLINE HOSE INSTEAD OF TOOLS SUPPLIED AT GARAGE. THE KNIFE SLIPPED CAUSING HIM TO SUFFER A OPEN WOUND TO HIS FINGER REQUIRING 2 STITCHES WHICH WAS QUAL IFIED AS SIMPLE REPAIR BY DR.
