He was unplugging the jaw conveyor about 5 p.m. and lost his balance and fell off. It did stated at one time that he jumped off. When he hit the ground he twisted his ankle. It did not hurt until the next morning, May 13, and was not reported until then.

EE BENT DOWN TO PICK UP SOMETHING AND HIS HARD HAT FELL OFF. BEFORE HE GOT IT BACK ON, HE WAS HIT ON THE LEFT SIDE OF HIS HEAD WITH A SMALL ROCK. HE HAD A HEADACHE, TOOK SOME ASPIRIN & RETURNE D TO WORK. HE WENT TO THE DOCTOR THE NEXT MORNING & RETURNED TO WORK.
EMPLOYEE WAS WALKING AROUND CRUSHER UNIT TO CHECK THE ENGINE OIL AND SLIPPED OR LOST HIS FOOTINGAND GRABBED THE FRONT OF THE RADIATOR TO KEEP FROM FALLING TO THE GROUND.
EE WAS MOVING SOME LARGE ROCKS BY HAND AND SMASHED HIS RT INDEX FINGER REQUIRING 4 STITCHES.
EMPLOYEE WAS ON TOP OF SCREEN DECK, HIS FEET WERE MUDDY AND HT EMETAL WAS WET. HE WAS LEANING OVER THE EDGE PUSHIN GON A WRENCH. THE WRENCH SLIPPED OFF AND THE EMPLOYEE FELL OVER THE EDGE LA NDING ON HIS LEFT SIDE IN A CURLED POSITION. HE WAS TOLD TO WAIT TO DO THIS JBO UNTI LEMPLOYEE RETURNED TO HELP HIM.
