Mostrando los 8EE reported to work on 6/25/2019 complaining of a sore and stiff low back. EE felt that working at the gravel bagging station the previous day may have led to a strain of EE's low back.
Employee went to the top floor of Dry Plant L to check the bin levels and tripped when entering the doorway. He lacerated the left thumb on the rotex during his fall. The rotex machine was inside the doorway.
The EE was manually lowering the dumper hopper attachment for the forklift and his index finger was pinched between the hopper handle and the forklift mast.
EE was lifting the end of a pipe to place it on the forks of the front end loader when he felt a strain in his back.
Employee was cleaning out a bagging machine. There was a hinge plate on the machine that fell on his finger causing a laceration.
When the operator, using a torch, attempted to burn an opening to install an idler on the conveyor belt, the flame apparently ignited soot build-up under the conveyor's rain hood from an oil burner that was used to heat the feed hopper. The flame then spread to a second conveyor. It appears both conveyors are destroyed as a result of the fire. Fire extinguished in 30-45 minutes.
Employee was filling and tying a 100# bag of sand. As he pulled on the top of the bag to lay it down on the conveyor, he felt a pain on the inside of his left elbow.
