Miner was using a grinder in the shop on 8/3/26. At 10:00 that morning they talked about their eye watering and was irritated. EE continued to work the rest of the day. 8/4/26 Miner came to work in the morning with an eye swollen. EE was taken to the local clinic to be seen for the eye. Clinic advised them to take the rest of the day off and to have a follow up appointment 8/5/26.

Hurt ankle on 2/12/25. Did not report injury till after EE went to ortho on 2/17/25. Claimed to step off haul truck and foot slipped and twisted ankle a bit 5 days later. Ortho found that there was a fracture in the foot. After our investigation EE was put on workmen comp for 24 days until EE was released back to full duty.
