In the late morning on 8/28/08, a small speck of metal got into EE's right eye. By 7 PM his right eye was very irritated and swollen. He went to ER and received medical attenion (metal sliver removed). No damage to eye. EE returned to work the following Monday, 9/1/08.

When the injury incurred, I told 2 co-workers. On July 28, 2008 I saw Doctor. He checked for inflamed muscle and took me off work."
EE suffered a left middle finger injury. Went to DR on 7/25/08, diagnosed a fracture finger.
On 6/17/08 employee notified supervisor that he had a very painful left shoulder. He advised us at that time he started to experience soreness in his shoulder a couple of weeks ago. He went to the local clinic on 6/17/08 to have it checked. He was taken off work pending further evaluation. The clinic referred him to another facility for tests.
Pulled back muscle.
Finger injured when rock popped back after going thru chopper/splitter. Reported to supervisor, Medical treatment sought immediately, medical treatment. **No sutures, glued to minimize scarring.**
tweaked his back and went home to rest. EE call in sick the week of 4/14, ask for work comp on 4/11/08. MSRQ is in the process of verifying the date, te name of treating physicians and what treatment received. MSRQ will also attempt to verify previous and present medical restrict if any.
EE was moving material around.EE picked up a piece of scrap metal abd while walking he slipped on some ice and twisted his back, but did not fall.EE told coworker he hurt himself, but did not feel that it was serious. Next day felt sore and pain, went to Dr and was informed he has piched nerve.
See attached co. will be sent to msha upon employee's return to work.
EE was stacking a pallet of rock when he was hit with a rock in the lower calf. When he got up he could not detect where the rock had come from. He told another EE & later his supervisor. He tried to walk it off but couldn't. Went to ER @ 2:30pm & was given CT Scan & Ultrasound, pain & swelling medication & told to Ice & Elevate leg. MRI scheduled & told not to work until cleared.
Stepped down out of forklift - experienced sharp pain in back.
I was operating the fork lift. Going backwards I was looking over my right shoulder. The forklift went over a large rock and bounced and jarred my back. The pain was terrible when it first happened. I let the company know about it later that day. I saw Doctor 9/5/07. He gave me muscle relaxers and said if it is not better in 7 days to call and make an appointment.
Lifting a rock to load on a pallet and EE felt right wrist pop. Saw doctor the day after, due to pain. Doctor took X-ray, not broken, sprained wrist.
While picking up rock to place in bucket, smashed finger against another rock.
Rovk rolled on hand breaking metacarpus of right hand.
