While trying to cement casing, ee was making cement, he clipped an open razor knife to his belt. When he bent down to get a bucket of soda ash, he cut his left forearm on open blade.
Rib slough in out first East lateral at 3500 dds intersection. Material came out of the rib 10'deep x 18'in length. Total area to repair will be 30'in length.
He was taking chuck out of drive tube, chuck was bound up. He was trying to broken loose the chuck with a pry bar when it slipped and smashed right ring finger. Stitches
Employee was attempting to insert a steel into a jackleg drill machine. The employee put his hand into a pinch point between the machine and the leg, and cut his finger.
As the night shift crew was exiting via the decline, they encountered a slough of ground from one rib 22'W x 12'D x 10'H. The toe of the sloughed material angled to the sill of the opposite rib. The crew decided not to drive their tractor any further and walked out 550' to portal. Ground was past some anchors.
He pulled a jackleg out of the drill hole
He was assisting miner shotcreting an intersection, and machine blew back dry shot crete and injured received dry shot crete in both eyes.
He was moving antifreeze barrels from the shop to the connex storage. He strained his back while moving a full barrel from a pallet into the connex.
Was exiting a backhoe, and struck head, while wearing a hard hat, on roof of cab.Received bilateral c4 laminar fractures. Employee did not report injury until 04/04/05, and then said he did not know how he had done it. Our industrial insurance carrier, AIG, denied the claim originally. This was just overturned by the Nevada State Industrial commission.
