Miner was lifting a roll of rubber hose into box of ORV and felt something pop in their arm. A co-worker helped them remove their coveralls, they were in a great deal of pain so the supervisor transported them to the local ER. Miner was examined, and an MRI was performed confirming a torn bicep muscle. Surgery is required to repair.

Mostrando los 9Miner was using a come-a-long to raise a take up pulley, the hook came loose and rebounded up between conveyor and toe-kick and glanced off miners cheek causing a laceration.
Miner was attempting to blow debris out of a flywheel assemble on the cone crusher through a small access whole in the cover. The nozzle either became clogged or EE inadvertently extended the tip into the opening making contact with the flywheel. This caused the nozzle to recoil cutting the palm of the miners hand resulting in a dr visit & stitches.
The Quality Control Technician was taking samples of finished material and strained her shoulder. The initial report was classified as non-reportable and has since been re-classified as reportable.
In the normal course of doing his daily duties, EE strained his left knee. Unknown exact cause of injury.
Fell from top of staircase of the crusher control house. Did three rolls down the steel staircase scraping both knees, bruising back and neck. No stitches required, employee released by doctor to regular duties as a supervisor.
Picked up a piece of metal to put in the metal bin. Smashed finger against the end of the metal bin. Received a laceration to the first digit of the left hand.
Cutting a piece of conveyor belt with a razor knife. The knife slipped and he stabbed his left hand. Employee received 4 stitches to close the wound.
Pouring concrete into an above ground form. 12 inches off the ground leaning on the mixer chute, when it moved he fell down on some form iron supports and lacerated his left lower leg. Employee received steri-strips to close and help heal wound.
