EE was cleaning the pit floor when a rock fell from the highwall and came through the windshield of the Komatsu WA600 loader EE was operating. Rock struck EE's right leg causing bruises/contusions, glass in EE's eyes, small lacerations.

Mostrando los 20EE was removing the radiator cap on the generator. EE thought EE had all the pressure released but when EE twisted the cap off it blew hot antifreeze on EE's back. Burns to EE's back.
EE was moving truck chains to the building and as EE was getting the chain on the hook, EE felt the middle of arm pop. EE then got into EE's car and drove to hospital where they diagnosed EE with a ruptured tendon in bicep.
Employee was stepping out of a pickup truck when EE's foot slipped causing EE to twist knee. EE worked the remainder of the shift but the next morning EE could not put weight on it and went for medical attention. The diagnosis probably an MCL sprain.
Victim was assisting with the replacement of a tooth on a loader bucket. Apparently EE's glove froze to the tooth and EE couldn't get EE's hand out of the way in time and EE's finger was caught between the tooth and mount. The tip of EE's left pinky finger was smashed between the two. The nail on EE's finger was removed
Slipped on ice.
Employee was prepping dozer for the upcoming shift. EE moved it to the fuel tank to fill the tank. The fuel tank is powered by a gasoline engine that is started with a rope pull. When EE yanked the cord to start the engine they struck a hand on the tracks because the dozer was parked so close. EE incurred a fractured 5th metacarpal on left hand.
Employee cut a tree. When the tree fell it struck another tree breaking the top out of the tree being cut. The top flew back and hit the employee.
Victim was assisting with extracting the highwall miner cars. EE had been shoveling dirt/mud off the deck of the miner. EE was standing at the rear of the deck while a car was being extracted and for unknown reason head came forward and was caught between the car and a support post. Suffered crushing injuries to the head.
Was climbing on bumper to check oil. Slipped on bumper on ice/mud. Hit shin, cut skin.
EE was using a hand held drill to clear dirt and other material from the hole that the pin goes into in order to connect two highwall miner cars. The drill bit hung in the pin hole and the drill twisted causing the injury.
Contractor was installing liner on the blade of dozer when a bolt broke that was supporting the metal plate. The plate dropped striking the contractor on the left foot. Result was a bruising and a fracture of minor bone in the left foot.
Victim is groundman/padman on highwall miner. Was using a hand operated drill with an auger bit. Bit got bound; handle on drill hit the back of employee's hand when it spun around. Two bones fractured by the impact (metacarpal bones leading to the index & middle fingers). Victim was taken to the hospital by the foreman; hand was put in a cast.
Victim was operating assigned loader. It had been raining a substantial amount. A rock fell from the highwall and struck the windshield of the loader causing dust, dirt & glass to get in EE's eyes. EMT's flushed and bandaged EE's eyes. At the hospital no foreign material was found in EE's eyes but there was minor scratches. Ointment was applied and EE was released.
The 2nd shift foreman, victim and another person were unhooking and draining the lines from the 8' water pump which had frozen. The water which came out of the water lines froze due to the -5 degree temperatures. Victim was holding onto the bed of the foreman's pickup when EE's feet slipped and EE fell on the ice. EE sustained a fractured left radius.
Victim was parking assigned dozer at the end of shift. EE stated to mine superintendent that EE was dismounting the dozer facing forward instead of backward and foot slipped off the push arm and went between the push arm and the track frame. EE suffered a fractured tibia just below the right knee. Surgery was done the next day to repair the damage.
We were notified on 04/13/2016 that employee had been ruled compensable for a black lung claim. Employee's employment at this mine ended due to a lack of work on 05/05/2015.
Fueler/greaser was greasing a Cat 980 loader from his service truck when the pressurized grease supply line ruptured discharging a stream of grease at pressure high enough to puncture the employee's skin and inject grease into his right thumb. The grease had to be removed by a surgeon.
Employee was welding and fell. Employee fell on 6/24/13, he didn't think he was injured and chose not to seek medical aide. On 6/26/13 he reported to his supervisor that he felt that he had damaged his ribs in the fall. There is no lost days - employee is on light duty.
Moving steel and rolled a T brace on left foot resulting in contusion. X-rays negative. Seven days restricted work. No lost time. Restricted work provided by employer.
