Injured employee had an in-line 4 cylinder water pump engine sitting on hardwood blocking material. While working on the engine it shifted and fell off the block and mashed hand.

Employee was cutting a hole in a piece of conveyor belting using a utility knife. The knife slipped out of the hole EE was cutting and lacerated EE's left hand. EE received stitches to close the wound. EE was released for full duty the same date of injury with minor lifting restrictions which did not prohibit EE from working full duty.

Employee exiting machine after trouble shooting; missed step on machine; landed on left leg / foot and sprained left knee

Injured employee was installing drive belts on the primary crusher when his fingers contacted the pinch point between the V-belt and pulley. Employee received a comminuted fracture on the tip of his pinky finger (Right Hand)and a transverse fracture on the ring finger of right hand.

Employee was dismounting a Caterpillar 769C haul truck when his left foot became wedged between the bottom of the bumper and the factory mounted hanging bumper step. Right foot slipped off the bottom step. This action caused the employee to lose his balance and 3 points of contact and he then fell to the ground. The doctor immobilized his leg and an MRI will be done on 10-4-13.

Employee was exiting a 10 wheel haul truck when the door handle broke off in his hand when he pushed down on the handle. The sharp edge on piece that was still attached to the door handle mechanism cut his hand. The cut required stitches to close the wound.

Employee was clearing out material by hand that had plugged the crusher. He got his finger mashed between the rock he was clearing and steel plate. X-Rays showed no broken bones. He was prescribed an anti-biotic and a pain reliever for the injury.

Employee was climbing out of a 18 ton stockpile truck when his foot slipped off the bottom step. His body shifted to the right and his left knee struck the door brace causing a knee strain and contusion. He was fitted with a moveable knee brace and was prescribed anti-inflammatories for swelling

Employee was rebuilding a water pump and applied a lot of pressure to remove the impeller. His left wrist was strained in the process and he was in pain. When he was seen by the physician, the injury showed possible signs of Carpel Tunnel Syndrome. He was issued a wrist splint to wear for two weeks and was placed with limited/restricted use of his left hand.
