Mostrando los 8Employee stated was attempting to locate a rattle on the 992 G End loader when employee stepped on a loose platform. This caused employee to lose balance and fall causing pain in right shoulder, right arm, right leg, and lower back. Employee was taken off work by a physician.
Employee stated that while climbing up into the 240 long boom excavator, employee felt a pull and sharp pain in right bicep.
Employee was attempting to fuel the D8 Dozer while standing on the crawlers. EE slipped and fell off the crawlers which caused pain in both legs. Employee was diagnosed with contusions and taken off work for 4 days.
Employee stated that EE was operating the dozer on the Raw Coal Stockpile when EE felt a pain in neck. Employee stated that EE felt pain when EE turned to look behind EE while backing the dozer up.
As employee was exiting the D10 Dozer at the platform, EE slipped and struck right knee against the frame of the platform. This caused a laceration to right knee requiring sutures.
Employee stated that while pushing refuse with a D9N Dozer, the dozer trammed over an offset in the material causing employee's neck to jar. this resulted in pain in employee's neck and right shoulder. The employee missed one scheduled work day and returned to work with a full release.
Employee was carrying a hydraulic jack and dropped it and attempted to catch the jack. When EE did, felt a muscle pull resulting in a torn bicep.
