Employee cut EE's fingers while changing the blades on the 950 Cat front end loader. EE was holding the plow bolt head with EE's right hand while using an impact with EE's left to loosen the nut. Once the impact engaged the bolt dropped down and spun cutting the 2 middle fingers.

Mostrando los 82 miners were removing journal rollers. While loosening the nut, the head of the bolt slid out of the hardware cavity. The injured minor used right ring finger to push up and reseat it into the cavity. When the 2nd miner engaged the impact wrench, the head of the bolt seated in the cavity and pinched the injured miner's ring finger, causing a laceration to their right ring finger.
Injured miner (IM) was transferring a plow assembly to a work table from an elevated forklift pallet to flip it off the pallet onto the table. The IM had left hand in the line of fire and the plow assembly landed on IM's hand causing an injury to the IM's left ring finger tip. Minor cut to the tip of the finger, and x-rays showed fracture that Doctor stated was old injury.
The employee was unfolding the radial stacker and had jumped off the ground to grab onto the bottom beam located about 3 feet over employee's head. While holding the beam the employee pulled self up and grabbed a sling with right hand to remove it. Employee popped the sling and lost balance causing employee to fall. The employee landed on right foot and fell onto back.
EE was throwing trash in dumpster when EE slipped on a wet area and fell, fracturing the right fibula.
While replacing the skirting on the bottom of the crude feed south blending hopper a rock fell from the grating above approx 11 feet and struck the employee on the head.
Using overhead hoist with chain and hook, a strap was used to grab hold of the plow body attached to the hook. The hook from the overhead hoist got caught onto an I-Beam which caused tension to build up on the chain lifting up Plow body which then slipped off the I-beam and caused the corner of the Plow Body to strike the employee on lower lip and bottom jaw.
Employee was lowering a metal plate to install at the mill floor that weighed 85lbs. The plate slid, cut and fractured employee's finger at the tip.
