Injured employee was helping another employee make a repair to a dozer with a pry bar. The bar slipped out of the other employees hand and struck the injured employee in the mouth resulting in a broken tooth.

Contract employee was pulling material out of the bed of a pickup, while up on the bed EE stepped on a piece of teflon material losing footing and fell off the edge of the tailgate landing on the cement floor. Cause of this injury was awareness. Compliance with rules/regs, PPE, mining equipment was not a factor, miner proficiency may have been a factor.
