while working on the 3rd floor of rail load-out building, the worker stepped backwards not realizing there was a hole opening in the floor. The worker fell through towards the floor below but caught self and was able to pull self up enough to roll out onto EE's stomach until help arrived. the hatch was left open by another company and not closed prior to work.

Mostrando los 3Worker was shoveling spilled material underneath an enclosed conveyor and, forgetting that they were under the conveyor, stood up quickly and hit the back of their head on a sample valve that extended out of the bottom of the enclosed conveyor.
Worker was cutting bags filled with barite above the hopper feeding the mill conveyor. While cutting the bags, the liner fell out of the outer bag (with material still in the liner) leading to the material banging the workers arm against the handrail.
