EXACT CAUSE UNK-OVER 7 DAY PERIOD WORKED ON MILL SPRINKLER SYSTEM,PICKED UP DOUBLE FLAP VALVE,PUT CHAIN ON ELEVATOR AND USED WHEEL BARROW.DETERM BY INS CO TO BE WORK RELATED INJURY ON 9-9-83.
LIFTING ON WRENCH TO ADJUST OVERHEATED HOIST BRAKE WHEN HE FELT SOMETHING PULL IN HIS LOWER BACK. REINJURY FROM ORIGINAL ACCIDENT 11/14/78. INJURY DETERMINED TO BE WORK RELATED ON 3/24/83.
HE WAS CLEANING BENEATH THE ELEVATED BAG FLATTENER IN SHIPPING AND WAS REPOSITIONING VACUUUM HOSE TO CONTINUE CLEANING WHEN A BAG OF TALC FELL OFF THE BAG FLATTENER, THE BAG FELL APPROXIMATELY 5 FEET STRIKING EMPLOYEE IN BACK OF HEAD.
A 2" X 1/4" WEAR PLATE WHICH SUPPORTS THE SKIP BAIL IN THE DUMPING POSITION CAME LOOSE WHEN A FAULTY WELD BROKE UNDER STRESS CAUSING THE SKIP TO BE STUCK IN THE DUMP POSITION FOR 55 MINUTES. T WO MEN RELEASED THE SKIP USING PRY BARS THE WEAR PLATE WAS REPAIRED, NO MEN WERE ON THE SKIP WHEN IT BECAME STUCK. NO ONE WAS INJURED OR ENDANGERED

INSPECTING EQUIMPENT AND FOUND PULVERIZER FEEDER OIL LEVEL LOW. WHILE PREPARING TO FILL FEEDER HE STRUCK HIS KNEE ON OBSOLETE MOTOR SUPPORT WHICH IS PART OF THE PULVERIZER THIS ACCIDENT ORGINA
