Front End Loader operator was approaching a pile of sand to fill bucket and place on the screen. The operator said he hit the pile of sand to fill the bucket and he felt pain in his elbow. He kept working and said that his elbow would hurt when moving it. He then reported the incidence to his supervisor. There were no witnesses to the incidence.

Mostrando los 9Front End Loader operator was descending ladder to take a break when he rolled left ankle as he stepped onto bottom rung. Ladder and operator's boots were both clean and free of debris. Operator stated he maintained three points of contact as he descended ladder. Operator could not recall looking at rungs as he descended. There were no witnesses to accident.
EE AND ANOTHER MECHANIC WERE INSTALLING A HYDRAULIC HOIST ON A TRACK BACKHOE. THEY HAD ATTACHED THE HOIST AT THE TOP BUT WERE HAVING TROUBLE WITH THE BOTTOM. THEY BOTH REACHED FOR A HAMMER THI NKING THE OTHER WAS HOLDING THE HOIST. THE HOIST MOVED AND STRUCK THE MIDDLE FINGER OF HIS LEFT HAND.
EE STATED THAT HE STARTED HAVING PAIN IN HIS LOWER BACK AND RT LEG 11-5-99. HE FELT THAT HIS PAIN WAS CAUSED BY BOUNCING WHILE OPERATING FRONT END LOADER AND REACHING OVER TO PICK UP TICKET MA CHINE FROM FLOOR OF CAB OF LOADER. WENT TO DR 11-8-99.
WAS GETTING OUT OF CAB OF LOADER HE GOT OUT OF THE CAB & TURNED AROUND ON PLATFORM TO CLOSE DOORHE STEPPED BACK TO CLOSE DOOR & FELL TO GROUND EE STATED THAT WHEN HE HIT THE GROUND HIS ELBOW WENT INTO HIS RIBS FRACTURING 1 RIB & BREAKING 2 OTHER RIBS
EE STATED THAT HE WAS RAKING CLAY BALLS OFF SCREEN OVER HOPPER. HE DISCOVERED A PORCELAIN INSULATOR STUCK IN BARS OF SCREEN. WHEN HE ATTEMPTED TO DISLODGE INSULATOR WITH HIS FIST THE BROKEN IN SULATOR GAVE WAY AND CAUGHT THE LITTLE FINGER ON HIS RIGHT HAND.
EE WAS STANDING NEXT TO DRAGLINE BUCKET HE WAS HELPING DRAGLINE OPERATOR. CHANGE. THE OPERATOR WAS ATTEMPTING TOSET THE CABLE WEDGE INTHE CLEVIS BY PULLING ON IT. WHEN THE OPERATOR PULLED ON THE CLEVIS, THE CABLE CAME OUT & STRUCK THE INJURED EE ON THE UPPER LIP.
EMPLOYE WAS PUSHING ON SCREEN T O LINE IT UP & FELT SHARP PAINS IN HIS BACK
LOADER OPERATOR WAS STANDING ON FRONT LEFT LOADETIRE. STEPPED ON TO FENDER AND LOST FOOTING STRUCK LEFT GRAB BAR OF LOADER WITH LEFT SIDE OF CHEST AREA.
