Removing the belt wiper on the conveyor feeding the gravel plant screen. While removing the 2nd bolt holding the wiper, the wiper fell down and pinned employee's hand by pinching it between the wiper and frame.

EE was changing our the screens on the gravel plant. The incident occurred while removing the 1/4" screen. He has continues to work, but was determined at a doctors appointment 5/7/15 that hernia would be repaired surgically in June.
While filling the fuel truck with fuel, the employee twisted his ankle on an eye bolt located in the walkway provided. The diagnosis was a sprained ankle.
The EE got off an end loader to turn off a conveyor belt. After running from the loader to the plant, he started to experience knee pain.
Employee was drilling with an 18 volt cordless drill. The drill bit caught and the drill twisted out of his hand and dislocated his little finger, right hand.
HE WAS PRYING THE DREDGE CUTTER CHAIN TRACK WITH A PRY BAR. THE BAR SLIPPED OFF THE CHAIN AND HE PINCHED HIS THUMB BETWEEN THE PRY BAR AND A PIPE.
EMPLOYEES WERE REMOVING FACE PLATE FROM DREDGE PUMP WITH CABLE HOIST. THEY WERE ATTEMPTING TO LAY FACE PLATE ON DREDGE DECK. THEY FAILER TO REMOVE A PIECE OF BOX IRON CLAMPED ABOVE THE PUMP. A S THEY LOWERED THE FACE PLATE, THE CABLE CAME IN CONTACT WITH THE BOX IRON AND BROKE IT LOOSE, CAUSING IT TO STRIKE THE VICTIM IN THE NECK AND SHOULDER AREA.
EMPLOYEE WAS GETTING OFF LOADER. HE SLIPPED AND FELL LANDING WITH HIS WEIGHT ON HIS LEFT FOOT.
REMOVING PIN FROM CUTTER HEAD PIN CAME FREE IT FELL ON FINGER SEVERING TIP
REMOVING HOLD DOWN CHAIN. PIPE SLIPPED AND HIT HIM ON HEAD.
EMPLOYEE WAS LIFTING SCRAPER PADDLE. IT SLIPPED FROM HIS HANDS AND HIT HIS FOOT.
THE INJURED WAS HOLDING COUPLING AS ANOTHER EMPLOPYEE WAS HITTING IT WITH HAMMER TO REMOVE IT FROM SHAFT.HAMMER BOUNCED OFF SHAFT HITTING HIS FINGER.
