EE was working at the cone crusher and said that a rock had hit EE in the back of the head. EE was then transported to the ER by a company employee, IE was awake and talking the entire time just complaining of a head pains. It was determined that EE had a small skull fracture and small bleeding on the brain, EE also had a small cut on the head.

HE THEN COLLAPSED TO GROUND AT THE BOTTOM OF LADDER. THE LANDING WAS CLEAR W/O ANY TRIPPING HAZARDS.
EMPLOYEE SAYS HE ACTED CARELESS AT THE END OF THE DAY. HE SAW SOME ROOT STICKING OUT OF ROLLER AREA IN CONVEYOR, THEN REMOVED GUARD AND REACHED IN TO PULL ROOTS OUT BUT ROLLER GRABBED HIS LITT LE FINGER AND IT WENT BETWEEN THE BELT AND ROLLER.
