Employee had replaced a fuel filter on a dozer during routine preventative maintenance. During this process, ee gloves became wet with diesel fuel. When ee attempted to stand up, ee hands slipped off of the hand rails. This caused ee to fall backwards and land on the left side of ee body, and the left side of ee head.

Employee was turning a nut with a wrench while performing routine maintenance on shaker. Employee slipped during this process and fell to the walkway causing lower back pain.

EE WAS FUELING A 980B CAT FRONT END LOADER WHEN HE SLIPPED OFF THE LOADER AND FELL TO THE GROUND. CARRIED TO THE HOSPITAL WHERE XRAY REVEALED THAT EE HAD BROKEN PELVIC BONE. SENT HOME TO RECOV ER.
