(Employee claimed the injury took place on October 2, 2013, but didn't report it until July 14, 2014). Employee claimed he was opening trapdoor to access crane deck. While opening the door made of deck plate, it came down on his head, bending head over to left shoulder.

Employee was using an etching tool when a metal fragment entered the eye. The item was removed and medication for the eye was prescribed. They received a full release to work.

Employee was performing house keeping duties near the primary feed when they presented signs of fatigue, they were given water and escorted to the first aid station where the decision was made to take them to the hospital. Individual was given IV fluids and released home.

Employee was moving fencing material, and pulled a muscle in groin. Employee did not report the issue for several weeks following accident. Diagnosis identified a muscle strain in the abdomen/groin area.
Placing 3/4" socket set into a cabinet in lower tool room when they felt a slight pain in their back.

The employee was injured on the 22nd of May and reported the accident on the 24 of May. The employee was taken to the clinic where a piece of metal was removed from his eye.

Employee walking to parking lot after end of shift. Employee stumbled and fell onto left hand. Resulted in sprain to left wrist and a slight contusion on left hand.
Individual completing pre-op inspection was mounting a forklift. While maintaining three points of contact and stepping up into the cab, the employee felt a pop in the right elbow. Resulting injury initially diagnosed as tear in right distal biceps tendon.

While working at the Heavy Equipment staging area, employee received a bug bite on lower left side of chin. Not exactly sure when it happened. Received medical treatment on Sept. 14th and was released the same day for regular work activity.

X-ray determined that a 2mm piece of metal is lodged in the outer edge of left ear. He has not decided to have the piece of metal removed as of 12-08-08
Laceration to finger
While attempting to walk around some water walking back from crusher employee slipped and fell onto right knee and then while attempting to stand up fell a second time on left shoulder.
While cleaning out tail area of feeder with water hose, materials sprayed back and some got into left eye.
