ICE ON GROUND CAUSED THE DRILL OPERATOR TO SLIP. AS THE EMPLOYEE TRIED TO GRAB SOMETHING TO BREAK THE FALL, THE DRILL GRABBED THE EMPLOYEE'S SWEATER AND PULLED THE EMPLOYEE'S ARM INTO THE DRILLING BIT.

While lifting a roll of cord onto loader, EE pulled lower Back employee claims. He also advised this is pre-existing condition and he works another job. (approx 20 lbs)
While performing pre start-up insp. on machine, operator walked around catwalk on excavator (245 Cat), catwalk broke and operator fell 5' to the ground, landing on his back and hitting his head. Operator was wearing hard hat.
While repairing the cable trac on the Novamac drill, operator fell approximately 7' to the ground. Strap broke causing operator to lose his balance.
