Driver slipped and fell on right leg and ankle while climbing into truck. Placed on light duty until 11/24/2014.
Bin truck driver twisted left knee at the end of his shift. Saw Dr. on 5-20-10 and was released back to duty driving truck. 5-27-10 follow up with Dr., MRI referral and restricted duty thru 6-1-10. 6-1-10 follow up with Dr., referral to orthopedic surgeon and relased back to duty driving truck. Will update 7000-1 following appointment with orthopedic surgeion on 6-21-10.
Miner turned/sprained his ankle while installing a screen. Miner did not report incident until Monday May 10, 2010. Miner stated he felt he was OK on May 6 and May 7 but his foot begin to swell up on Saturday May 8 so he sought medical treatment over the weekend and received a gel splint to help immobilize the foot. Miner was released back to full duty the same day.
12-6-07 employee slipped on ice straining his left knee. Employee did not seek medical treatment. 12-11-07 employee reported that knee was not improving, supervisor took him to local clinic. Employee was examined, diagnosed with knee strain, released same day, told he could continue to operate water truck, but should undergo 4-8 sessions of physical therapy.
Employee strained lower back while cleaning the flywheel on the shorthead cone of a portable crusher. Employee said he was OK and finished his shift. The following day he went to a doctor and was taken off work for one day and returned to work on Aug 16, 2006. Doctor diagnosed the injury as a simple muscle spasm.
Employee strained his right shoulder while exiting the cab of a 950F front end loader.
Employee was assisting with removal of the outer sleeve from an impact bar for a portable crusher. Bar was removed and placed on the ground. Upon spining bar over to allow a cut to be made for removal of outer sleeve, employee lost his grip & bar pinched his finger between bar and his steel toe boot. Sutures required.
Employee stated that his back was sore. He does not know why. Employee visited a doctor on 12/16/2005. Doctor suggested that short term physical therapy may help.
Control Room Operator strained lower back while lifting a jackhammer. Operator did not notify this office of the injury until October 3, 2005. Employee went to his doctor. Employee was released back to work the same day with lifting restrictions and referred to PT. PT says that restrictions will not affect the employee's ability to perform his normal routine duties.
Individual was moving a 55 Gl. drum and loss control of the drum causing a shoulder strain.
EE HELPING WITH SET UP OF CRUSHER. IT WAS VERY WINDY. THE WIND BLEW SOMETHING IN EE'S EYE. EE HAD SAFETY GLASSES ON.
EMPLOYEE TRYING TO LOOSEN NUT WITH WRENCH AND SHEATER BAR. NUT CAME LOOSE AND CHEATER BAR STRUCK EMPLOYEE IN THE CHEST.
