Employee found debris on the under belt scraper of the head pulley coming out of jaw crusher (7158) and attempted to clean it without de-energizing the piece of equipment first. This resulted in the belt grabbing the 3 foot pry bar and slamming EE's hand into the scraper and framework of the under belt. This resulted in 2 broken bones in the Right arm and hand.

Mostrando los 24Replacing a screen. Lifted screen to remove a rock and screen slipped from coworker's grip causing screen to fall on employee's hand. Lacerating left palm. Required stitches and tetanus shot.
Employee was repairing the decking on a drop deck trailer upon which a generator sits. To access the decking a small transformer needed to be removed. While attempting to remove the transformer it fell striking the employee's left pinky finger lacerating it. Gloves were being worn.
Was walking across yard slipped on snow covered ice. He had a quad tendon rupture in his left knee.
Injured employee was standing next to a parked skid steer loader talking to emp. sitting inside the skid steer loader. IE turned to walk away from skid steer loader & tripped on the loader's bucket which had a 4' extension attached to the bucket. IE tripped & fell to ground producing a fracture to rt 5th finger. Positive x-ray diagnosed on 8/03/2011. IE initially refused treatment.
The employee was using a spud bar clearing rocks from the cone crusher when his left little finger contacted the threads for the cone adjusting ring. The threads cut through the employees glove and lacerated the second knuckle on this finger. The laceration was cleaned and sutured at the walk-in clinic.
The employee was attempting to couple a 6 inch discharge hose onto a pump with cam lock fittings and strained his back. The employee reported the injury but did not request medical attention until May 3 at which time a physician was seen and the employee was release to restricted duty which the employer has made available.
Using pry bar to push punch plate - plate slipped off bar hitting tip of finger.
Employee placed middle and ring fingers on adjustment rod for top curtain hanger on HSI frame. The HSI curtain was not completely loose and was being held in place by a rock. When the rock broke free, adjustment nut slid down onto the adjustment rod pipe pinching the 2 fingers.
Employee slipped and fell. He caught himself with outstreched arm, dislocating shoulder.
During cone liner change; employee bumped knee, opening an old incision on knee. Stitches were required to close.
Superintendent was wrenching on an adjusting nut on the conveyor tail pulley. The wrench slipped causing his right hand to jam into the metal guard and lacerating the end of the middle finger. Xrays revealed a fracture in the end of the finger as well.

EE WAS WALKING NEAR STORED CONVEYORS IN STAGING AREA ON FLAT GROUND. FELT LEFT ANKLE "POP" AND FELL TO THE GROUND. ANKLE BECAME SORE, STIFF AND SLIGHTLY SWOLLEN.
Laborer prepared a piece of angle iron to cut with cutting torch. As soon as he put heat to it a foreign object popped and hit his cheek under his safety glasses and the object fluid p in his right eye. shoulder had a face shield over his safety glasses or welding goggles.
EE WAS IN SCREEN, HE WAS BEING HANDED PIECES OF FLAT METAL. PIECE OF IRON CAUGHT HIM IN THE LEFT EYELID - 3 STITCHES - PROTECTIVE EYEWEAR WAS PROVIDED BUT NOT WORN.
EMPLOYEES WERE CLEANING OFF EQUIPMENT TO MOVE, A ROCK FELL FROM A GUARD STRUCK THE EMPLOYEE IN THE MOUTH CHIPPING HIS TOOTH. THE EMPLOYEE SAW A DENTIST RECENTLY AND IT WAS DETERMINED HE WOULD REQUIRE A CROWN.
WHILE DRILLING A 1/2" HOLE IN METAL USING A DRILL PRESS, THE DRILL BIT CAUGHT & SPUN METAL. THE SPINNING METAL CAUGHT THE GLOVE, PULLING IT DOWN & LACERATING A 1" SLIT IN FOREARM.
EE WAS ADJUSTING TRAP BELT WHEN A FROZEN CHUNK FROM THE GRIZZLEY ROLLED DOWN THE BANK AND BUMPED THE BACK OF THE EE'S LEGS, CAUSING PAIN IN HIS BACK.
LIFT AND JERKING A ROCK GUARD (BASKET).
WAS TRYING TO SEPARATE ROCKS WITH A METAL BAR & SLIPPED & HIT THE LEFT SIDE OF THE FACE BELOW THE EYE. HAD TO HAVE STITCHES.
EMPLOYEE WAS WALKING DOWN A GRAVEL PILE AND STARTED TO SLIDE. WHEN HE CAME TO A HALT ON STABLE GROUND, HIS KNEE ON THE RIGHT SIDE GAVE OUT AND HE FELL TO THE GROUND. POSSIBLE RIGHT-KNEE INJURY .
EMPLOYEE WAS PLACING A BIT IN A DRILL WHILE PLUGGED. THE TRIGGER STRUCK THE COUNTER TOP AND THE DRILL SPUN WHILE THE CHUCK WAS STILL IN PLACE. THE CHUCK SPUN UP AND CUT THE EMPLOYEE'S FINGER.
THE EMPLOYEE WAS CUTTING A BENT HYDRAULIC CYLINDER OFF OF A GRIZZLY WHEN HE CUT THROUGH THE CYLINDER IT SPRANG OUT AND STRUCK HIM IN THE LEFT FOREARM CAUSING A FRACTURE.
