Mostrando los 16Employee was carrying a 48 inch pipe wrench with his right hand and a 15# hammer on his left shoulder. EE started to bring down the hammer and lost control and the hammer fell onto his right hand that was holding the pipe wrench. EE's has a fracture metacarpal on his right hand. EE has been put on light duty and his next doctors appointment is in 10 days.
IE (Winchmen/Supervisor) yelled to stop another EE from cutting a hole in the super structure, EE (maintenance) yelled back and said he was in charge. IE stated that the other EE threw a bolt at him (not hitting him) and IE was upset and turned and hit the control room wall with his left hand.
Working on a metal deck with mud and wet conditions. This could be preventable by washing decks as needed.
Taking apart two rubber hoses, king nipple inside one hose was worn and had sharp edges that cut ee left arm.
EE was working on a vehicle and after completing his task he stepped back and lost his balance. Trying to catch his balance he landed on his elbow.
While operating our D-9 Cat removing down tree the cat rolled up on a log and then rolled off causing the Cat to travel fast and hit hard throwing operator out of seat then hitting operators head on bar on cab cutting his head.
While using a pry bar to re-install the 1/2" cable on the dredge winch the pry bar slipped causing the EE to hit his left leg on the deck.
While machining upper tumbler bolts, the long sleeve of shirt got tangled in bolt and was pulled into lathe. Hand hit threads, cutting left pinky finger and breaking it. It spun employee around and he hit his back and left ear.
Went home not feeling good. Called 3 days later by his dad and said he was in the hospital with foot infection.
Injured at shift change while stepping into launch's plywood floor, caught foot in floor, tripped and twisted his knee.

EMPLOYEE WAS FILLING A HOLE IN ROAD WITH SAND. HE USED HIS RIGHT FOOT TO SMOOTH OUT THE SAND AND TWISTED HIS RIGHT KNEE. (THE SAND WAS DRY)
EMPLOYEE WAS WALKING DOWN STAIRS WHEN HE SLIPPED AND FELL BACK. HE TRIED TO CATCH HIMSELF AND LANDED ON HIS LEFT WRIST. NOTED TO EMPLOYEE TO SLOWDOWN GOING DOWN THE STAIRS.
