THE REMOTE CONTROL CONTINUOUS MINER WAS BEING OPERATED IN THE FACE OF #9 ROOM OF THE 0 SOUTH OFF 1ST EAST UNIT#002-0. AT APPROX. 6:25 PM A MASSIVE ROOF FALL OCCURRED FATALLY CRUSHING THE MINER OPERATOR DANZIL GOINS. THE ROOF FALL WAS APPROX. 50 FT LONG, 14 FT WIDE, 0" TO 4FT THICK.
THE VICTIM WAS DRIVING A GROVE RT 59S MOBILE CRANE, WITHOUT A LOAD, ON THE SURFACE MINE'S MAIN HAULAGE ROADWAY. THE CRANE ROLLED ONTO ITS LEFT SIDE IN THE MIDDLE OF THE ROADWAY, CRUSHING THE O PERATOR IN THE CONFINES OF THE OPERATOR'S COMPARTMENT. THE ACCIDENT OCCURRED ON A LEVEL ROADWAY THAT WAS STRAIGHT AND APPROXIMATELY 70 FEET IN WIDTH. THERE WERE NO EYEWITNESSES TO THE ACCIDEN
I,NAME ATTEMPTED TO DRIVE HIS SERVICE TRUCK THROUGH 2 WASHED OUT HAULAGE ROAD. THE WATER CARRIED THE OFF OF THE ROAD AND INTO A STREAM. I,NAME WAS DROWNED ABOUT 5 HOURS LATER THEY FOUND HIS BO DY DOWNSTREAM.

THE VICTIM CLIMBED, FELL, OR WAS THROWN FROM THE CA B OF THE DOZER,3HE THEN FELL THROUGH THE HOPPER, ONTO THE FEEDER BELT AND CARRIED TO THE 2 WASHBOX WHERE HE WAS DISCOVERED.
ON 2-20-87 AT 9410 AM IN THE 3 LEFT OFF 2 SOUTHWEST SECTION IN #2 ENTRY OUTBY SURVEY STATION 1777 A LARGE SECTION OF MINE ROOF 50' LONG 20' WIDE & 15' HIGH FELL ON A JOY 12 CM C/M RESULTING IN FATAL HEAD INJU9IES TO THE C/M OPERATOR.
APPARENTLY WAS CHECKING SLOPE BELT WHEN HE BECAME ENTANGLED IN THE IDLER ROLLER TO THE BELT TAKE-UP CAUSING A FATAL INJURY
THROWED A BUNDLE OF PINS UPON PINNER. STEPPED IN AHOLE FELL INTO SHARP CORNER OF PINNER. JABBING SELF IN NAVAL AREA.
