EE WAS PUTTING A H.V. ELECTRICAL CABLE OVER A 90' HIGHWALL. THE CABLE WAS LAID FROM THE ELECT. BOX AND THEN PARALLEL TO THE WALL ABOUT 200' AND 35' FROM THE EDGE AND THEN WAS LAID INTO LOOPS A ND WAS FED OVER THE WALL. AFTER ALL THE LOOPS WENT OVER, THE CABLE THAT WAS PARALLEL SNAPPED TO THE SIDE, STRIKING EE WHO JUMPED TO MISS IT. EE WAS INSIDE OF A LARGE BEND.

EE WENT TO DR ON 9-21 AND WAS DIAGNOSED AS A B RUISE BECAUSE FINGER STILL BOTHERED HIM. EE WENT BACK TO DR. ON 9-30-96 AND IT WAS DIAGNOSED AS A FRACTURE.
YEMPLOYEE WAS HELPING PUMPER MOVE A PUMP LINE. WHILE DISMOUNTING FROM A TRACK DOZER, EMPLOYEE SLIPPED AND TWISTED BUCK. EMPLOYEE WORKED UNTIL 9-26-96 WHEN HE STARTED TO MISS WORK DUE TO BACK PAIN.
EE STATED THAT HE WAS DISMOUNTING FROM A SPRINKLE TRUCK. FOOT GOT TANGLES IN A CABLE STEP AND HE SLIPPED AND FELL INJURING THIGH, BACK AND RT KNEE. EE SAW HIS SUPERVISOR AT END OF SHIFT (11PM) AND SAID NOTHING ABOUT FALLING. EE WAS DISCIPLINED EARLIER IN THE SHIFT FOR SOMETHING.
EMPLOYEE WAS PINNING HIGHWALL MINER CARS TOGETHER. WHEN HE PUT ONE OF THE PINS IN, HE SMASHED HIS THUMB BETWEEN THE PIN AND THE CAR.
A 1370DRAGLINE IS BEING USED TO DIG A TRENCH CUT FOR A HIGHWALL MINER OPERATION. THE DRAGLINE HAD FINISHED DIGGING MUD BEHIND IT AND SPOILING IT AND WAS PARKED AFTER THE 7-14-96 2ND SHIFT. NO CREW ON THE 11-7 SHIFT. THE DAYSHIFT CREW DISCOVERED THAT MUD FROM THE SPOIL ON THE SIDE SLIDE IN AND COVERED THE RT CAB, THE FRONT OF THE MACHINE, AND SOME OF THE RT SHOE. DAMAGE TO RT CAB, L
EE WAS PULLING ON MINER ELECT. CABLE TO LAY IT IN A TROUGH. AS EE WAS DOING THIS, HE BUMPED HIS ELBOW AGINST THE EDGE OF A HIGHWALL MINER CAR. HIS ELBOW WAS BRUISED AND WITHIN A FEW DAYS HAD T O HAVE FLUID DRAINED FROM IT.
EE WAS USING A PRY BAR TO CLAIGN 2550 DRAGLINE BUCKET RIGGING. AS HE WAS PULLING ON THE BAR, THE BAR SLIPPED CAUSING EE TO TWIST RESULTING IN LOWER BACK PAIN.
EE WAS DISMOUNTING FROM THE 1ST ADDCAR. HE STEPPED ON THE RUBBER FLASHING ON THE SIDE OF THE CAR PART OF THE DUST CONTROL THINKING IT WAS METAL. THE RUBBER FLEXED DOWNWARD AND EE FELL ABOUT 3 1/2-4' TO THE LAUNCH WALKWAY AND THEN ABOUT 3 1/2 TO THE GROUND. THE RUBBER FLASHING WAS REMOVED TO PREVENT A RECOURRENCE.
GIBCO TRUCKING DRIVER WAS TARPING HIS SEMITRUCK ALONG MINE ACCESS ROAD. WHEN HE ROLLED THE TARP OVER FROM GROUND LEVEL, A FEW TIE DOWN STRAPS DID NOT COME OVER THE SIDE. THE DRIVER CLIMBED UP ON THE SIDE OF THE TRAILER TO THROW THE STRAPS OVER AND FELL OFF. THE DRIVER COULD HAVE REACHED THE STRAPS FROM GROUND LEVEL USING A METAL POLE ON THE TRUCK.
EMPLOYEE CLOSED DOOR AND WHEN SHE UNLATCHED THE DOOR, THE DOOR SHUT WITH HER RIGHT THUMB IN THE DOOR JAM.
EE WAS CUTTING A MUD FLAP FROM BELTING MATERIAL USING A DRAW KNIFE. WHILE HOLDING A STRAIGHT EDGE IN PLACE WITH HIS RIGHT HAND AND CUTTING THE MATERIAL, THE UTILITY KNIFE SLIPPED AND CUT HIS R IGHT HAND.
EMPLOYEE WAS IN PROCESS OF PINNING CARS TOGETHER. PIN WAS HARD TO GO IN AND WHILE REPOSITIONING CAR, PIN SUDDENLY DROPPED, MASHING ENDS OF TWO FINGERS AND JERKING EMPLOYEE'S SHOULDER.
8" RAINFALL OCCURRIN GON 4-28- AND 4-29 CAUSED THE WEST FIELD TO FLOOD. THE HHIGHWALL MINER AND RELATED EQUIPMENT WERE FLOODED APPROX 25 FOOT OF WATER IN THE PIT. PUMPING ACTIVITIES ONLY BEI NG DONE MIENR RECOVERED ON 5-7-96. OPERATIONS IN PIT TO HOPEFULLY RESOME TO WEEK OF 5-13-96.
EMPLOYEE HAD JUST DISMOUNTED FR9OM A HYDRAULIC LOADER. WHILE STANDING NEXT TO THE LOADER, EMPLOYEE EXPERIENCED SEVER PAIN UNDER THE BELLY BUTTON. DIAGNOSED AS A HERNIA. NO SLIP OR FALL.
EMPLOYEE WAS ASSISTING IN REPLACING A HAULAGE TRUCK STARTER. THE STARTER WAS PULLED INTO POSITION USING A COM-A-LONG AND ROPE. THE ROPE BROKE AND EMPLOYEE PUSHED STARTER AGAINST FRAME. REIN JURING WRIST. STARTER WEIGHED 50 LBS. NOT INSPECTED BEFORE USING BUT EMLOYEE SAID IT LOOKED OK. RE-OCCURRANCE OF INJURY FROM 2-22-96.
A DISCHARGE HOSE OF A DIESEL PUMP HAD ICE & SLUCH IN IT. EE WAS USING A 10# SLEDGE HAMMER TO BREAK UP THE ICE & INJURED HIS BACK.
WHILE SERVICING A TRACTOR, EE GRABBED A HANDHOLD TO PULL HIMSELF UP ONTO THE TRACTOR. WHEN HE DID, HE PULLED THE LIGAMENTS IN HIS HAND. EE DID NOT SLIP OR TRIP.
EE WAS DISMOUNTING FROM A FRONT END LOADER. WHEN HE PLACED HIS RIGHT FOOT ON THE GROUND, IT WAS IN AN AWKWARD POSITION (TOE FIRST) AND EE FELT A SHARP PAIN IN HIS CALF. HANDHOLDS AND STEPS WER E IN GOOD CONDITION.
WHILE DOING MAINTENANCE ON 558 D11 DOZER, THE EMPLOYEE WAS USING A 3/4" RATCHET WITH A EXTENSION TO BREAK OPEN A DRAIN PLUG. THE EMPLOYEE PULLED AND WHEN THE PLUG BROKE LOOSE HE HIT HIS LEFT ELBOW ON THE TRACK GROUSER.
