SCOOP OPERATOR HAD BEEN SCOOPING RIBS, HAD STARTED TO CLEAN THE CORNER AND WAS PARKED. IE STEPPED INTO THE SCOOP BUCKET TO TELL THE OPERATOR EE WAS NEEDED ON THE OTHER SIDE OF THE SECTION. A PORTION OF CAP ROCK BECAME DISLODGED, FELL INTO THE BUCKET AND STRUCK IE's LOWER LEG.

Mostrando los 10Employee was bolting top on the operator side of a Fletcher Bolter. While bolting, a piece of rock fell crushing the tip of the employees fifth finger on the right hand.
Rock fall was discovered on fire boss run by mine foreman. The fall was in #4 face extending across the face through intersection to halfway between 5 + 6 headings.
First cut in # 4 entry. Developing barrier. Was finish and awaiting the bolter when the fall occured. Fall extended from the fresh cut and 16 ft outby pulling approx. 4 - 5 rows of roof bolts. No miners or equipment were damaged in fall.
Had a roof fall above anchorage in No. 7 Entry one break inby survey station 258. There was a slick sided slip that turned loose. 18' wide - 50' long - 7' high. This fall was not in face and did not hinder travel to and from face area.
An unintentional roof fall occured in the #2 entry. Approximatly 100 feet out by the fall the fall was above bolts average. No one was injured on this occurence.
Upon mining the #2 heading. The mining machine cut into an auger hole. The auger hole was determined to contain no water or gasses. No auger holes were projected on mine maps. No injrues occured as a result.
EMPLOYEE WAS TRAVELING THE MINE HAULROAD IN HIS PERSONAL VEHICLE WHEN HIS VEHICLE WAS STRUCK BY A COAL TRUCK.
COAL TRUCK FAILED TO STOP AND HIT PICK UP COAL TRUCK.
TRANSMISSION FAILED WHILE IN 2ND GEAR AND DRIVER JUMPED FROM VEHICLE. TRUCK PROCEEDED 50-60 DOWNHILL. STOPPED AND ROLLED OVER WHILE STILL DOWN GRADE.
