Had hand resting on canopy of bolter rock fell out struck him on the pinky finger and broke it. Employee improperly placed hand on canopy.
Removing pinner steel from the roof a rock broke loose and fell and hit him on the cheek cutting it put 8 stitches. Employee failed to scale loose rock.
Employee was retrieving a rope and chain from under a rock that was on the ground. When the rock fell forward and caught the employee and pushed him to the ground. The rock ended up on his lower legs and back.
Employee stated he was doing repair work to the feeder on #4. He was cleaning the feeder flights and bolts holes out as he was laying them on the side frame of the feeder. At this point, one feeder flight slid off the feeder and struck the employee in the left lower.

Employee was struck by rock while cleaning out his dust box in the #5 entry on #1 section. The employee received a linear non-displaced fracture to the distal fibula. The employee was first given a full release but on 4/30/14, information was received on the fracture.
Employee stated a rock fell a short distance while he was on top of the continuous miner repairing a water hose going thru the belly of the machine. The employee continued to work but on 5/5/14, doctors placed him on restricted duty. As of 5/19/14, doctors have him off work.

Employee stated he was bolting in the #5-left x-cut when a rock fell and struck him on the upper right arm causing a laceration that required 11 sutures. The rock measured 19" by 11" by 1" to 1" in thickness
Employee stated he was walking to get a golf cart tire off the tire rack. When he turned to walk away, he tripped over a piece of angle iron and fell to the ground. Employee continued to work until 4/2/2014 when doctor reported he had a broken left #6 rib.

Employee was examining belt lines and was exiting her golf cart from nearly a prone position and twisted her knee. She continued to work until 5/2/14 when she had a medical procedure done on knee.
Employee stated he was pulling up miner cable with left arm and felt pain as he was pulling on cable, in his lower back.

After further consideration and because the Insurer found the injury compensable, this is being reported as a lost time injury.

C Panel, #7 entry, spad 11+80 off the Blueberry Hills submain out by section appr. 1000ft. A rock fall was found measuring appr. 19' by 20' by 6' plus high.
A rock measuring 4ft by 3ft by 3in thick fell from between pins and struck employee in the right leg above ankle.
Employee was in #6 entry behind his roof bolter when the miner pulled through the wing curtain and struck the employee in the right buttock.
EE was repairing oil leaks on a truck that is equipped with many accumulators. An attempt was made to release all of the system's stored pressure. There were 3 hoses that already been repaired and no pressure was present. While removing the 4th hose, stored pressure was present, causing hose to fly into EE's face and strike him in lower eye, had safety glasses on.

There is no specific incident related to this illness. The EE believes after conferring with his Dr. because he is often on his knees, it could be that due to grinding dirt into his skin and infection developed. Although he first missed work 10/12/10 he did not report to the mine it was work related until 11/12/10.

Six employees traveling 90 day primary escapeway. The covered mantrip that they were traveling on rubbed a roof bolt and the back cover was pulled loose. The SCSR storage box mounted under the cover struck an employee in the face and right eye.
Mantrip at 2E turn on travel road had stopped to communicate with person at scoop charger. The mantrip was put in park and operator left mantrip. The mantrip slipped out of gear and rolled forward and struck employee in the left hip.

The employee was installing roof support to the roof when a rock (18" by 10" by 0.0" to 0.5) fell and struck him in the upper left arm. #1 unti, #7 room, just inby 12+60 spad number. First aid was given to the employee and he was taken outside for further treatment. He was given 5 stitches.
