VICTIM ENTERED A VERTICAL SHAFT THAT WAS BEING USED AS A WATER SUMP. SHAFT WAS 90\ DEEP 7\ 3" IN DIAMETER, CONTAINING 28\ OF WATER VICTIM WAS APPROX 15\ - 20\ DOWN THE LADDER,. WHEN HE APPAREN TLY LOST CONCIOUSNESS DUE TO A LACK OF OXYGEN AND FELL TO THE BOTTOM OF THE SHAFT
