Employee was barring out the cone. Material gave way suddenly and employee hit arm against the cone. Received x-ray after continued pain and was diagnosed with a fracture. Reportable as of 3/30.

EE was using the power washer to clean the concrete area where the water truck fills up. It is believed that EE was spraying off EE's work boots (an unsafe act) when the hot pressurized water entered the top of EE's boots and hurt EE's ankle.

Employee worked all day, but, around 3:00pm, complained of not feeling well due to the heat. Employee went home and began throwing up, then went to the ER for the heat related illness. Lost time as of 7/8/2021.

The injured employee was preparing to mount the cab of a flatbed truck. When EE placed EE's foot and tried to hoist self up, EE's leg twisted, which caused EE to fall. EE sustained a knee injury and was diagnosed with a sprain. Lost time as of 5/18/2021.

Injured employee was assisting with changing screens with another employee at the wash plant. In the process, the employee caught EE's finger on the sharp edge of the screen. EE's finger was lacerated and required 7 stitches. No lost time.

The miner was dismounting loader in the lube bay when the miner stepped on a fuel hose. This caused miner's knee to buckle and miner to fall to the ground. The miner sustained a knee sprain.

The miner was tapping out a sheared bolt on a motor housing and using PB Blaster to tap it out. When that didn't work, the miner began to torch the metal to loosen it. The residual PB Blaster flashed at the miner and EE received 1st degree burns to EE's face and neck.

The employee was dumping a wheelbarrow full of sand when the wheelbarrow tipped too far forward and caused the employee to strain their midsection in the process. The employee worked until 7/30/2018, when the injury became a lost time accident.

The employee was performing a workplace exam at the wash plant. When the employee stepped up and walked on a concrete platform for a piece of equipment, employee's foot slid out and employee almost did the splits. Did not become a lost time accident until 4/12/2018.

The miner was operating a crushing spread. The miner went to the top deck of the cone crusher to check if the cone was operating normally. The miner didn't follow proper procedure in this case. The miner looked into the cone when there was only a few rocks in it and when EE did, a rock bounced out and hit EE in the nose. The miner was seen at urgent care for a nose laceration.

Employee was changing a fuse in a bucket inside of the Wash Plant trailer (MCC). The employee shut down the bucket, opened the bucket, reactivated the bucket, removed and replaced one fuse and upon removing the second fuse, an arc occurred. The employee received minor burns to hands as a result.

The employee was using a two-handed sledgehammer to move material in a feeder by striking the sides of the feeder. As the employee swung the sledgehammer over his head, he felt a "pop" in his left shoulder and released the sledgehammer, which then struck the same shoulder (left). The employee has scheduled an MRI to determine the extent of the damage (an x-ray revealed no damage).

The employee was adjusting a spring on a screen. While trying to get it into position the spring shifted cutting off the tip of his thumb between the spring and the cup. He was taken by us to hosiptal where they looked it over and wrapped it and said it will heal on its own, no stitches required.

EE was using D8T Cat. Dozer w/Semi-U dozer blade w/approx 3050 total hrs on machine. He failed to stop machine & reverse unit to climb back up slope he'd just dozed the material from. His failure to control machine resulted in pushing through safety barrier & causing machine to become upset from its normal operating position. Received cut to left hand.
