Employee was working and was overcome with heat illness. Was seen in emergency room. Loss time accident as of 6/9/2022.

The employee was working at our Northern plant, running a crushing plant, when the supervisor noticed that EE began acting a little off. Thinking it was heat related, supervisor ushered EE to the scale house to sit in the air conditioning. EE was given water, which EE proceeded to vomit up. EE was then taken to the hospital for evaluation.

Southwest Rock Products · Absorption of radiations, caustics, toxic and noxious substances

The employee was assisting with cleaning up spilled anhydrous lime around the crushing and screening plant. During the course of this process, the anhydrous lime came into contact with the employee's wet skin, causing 1st degree burns. Lost time as of 8/6/2019.

The employee was using a large bar to knock a large rock out of the cone crusher. The employee was putting a lot of force behind the bar, when the rock suddenly moved, pinching EE's finger between the bar and the side of the cone. Lost time as of 8/2.

The employee was stepping up onto a platform and lost balance. When employee's leg came back down to stabilize self, employee was cut by a stray piece of metal. Lost time as of 8/2.
