On 2-7-24 between 5 and 6 am. employee was checking the 57-rock stacker during routine monitoring of plant. EE noticed that belt #1 feeding the 57-rock stacker had stopped. EE started making way to the control panel to shut off belt to prevent overflow. On EE's way to the control panel to shut it off, EE stepped wrong hurting their left foot around the achilleas area.

The employee was operating a 988 loader when EE's back began to hurt. EE was diagnosed with a back sprain. Lost time as of 6/7/23.

Employee was on EE's knees shoveling. When EE finished, EE put EE's hand down to help self up and contacted a sharp rock with EE's hand, resulting a cut that needed stitches.

The employee was shoveling by the chute of a sand and gravel screen when a rock bounced off the chute and hit the employee on the hard hat. The employee complained of neck pain and was seen at an urgent care. Loss of time as of 3/19/21.

The employee claims to have been shoveling 3/8 rock into a wheel barrow while cleaning up the plant. Upon lifting the wheel barrow full of 3/8 rock, the employee felt "tearing and pain" in the abdomen. The employee was later diagnosed with two hernias.

Employee is a loader operator. When the air conditioning in the loader was unable to be repaired, the employee elected to run the loader (unbeknownst to Supervisor). Towards the end of the day, the employee complained of not feeling well, was seen in the emergency room and diagnosed with dehydration. Employee returned to work 8/14/17 (no lost time).
