Arcosa Aggregates West, LLC. · Struck against a moving object

On 7-3-24, employee woke up with a sore back. During the shift, they operated a 980m Front End loader. About halfway through the shift, they got off the loader to work on the wash plant. They got back in the loader and around 10:00 a.m., they informed the Plant Manager of a minor pain in their back.

The employee was airing up/checking the pressures of the tires of the water truck. After checking the pressure of one of the tires, the employee went to grab the air chuck to add air to the tire. As EE approached, the tire blew out. The employee was about 4 feet away from the tire, but the air pressure blew EE's safety glasses against EE's eyes and debris into EE's eyes.

On 2-26-24 EE was using a MAG drill to drill holes on the inside frame of a stacker. In operation EE attempted to clean the metal that was stuck to the drill bit. The drill bit caught EE's glove entangling EE's hand into the drill bit. An employee that was assisting hit the emergency stop on the drill. The entanglement resulted in a laceration to right hand.

EE was on top of a conveyor hooking up chains to the lifting eyes to lift the conveyor with the 988k Loader. EE signaled to the loader to put pressure on the chains and then unhooked the fall protection to be able to climb down. The chains that were attached then slipped causing the employee to fall off the side of the conveyor 10 feet to the ground.

The employee was walking to the port-a-john when plastic (trash) wrapped around their leg and tripped them. They hit the gravel causing cuts to their hand and forehead. No lost time, but sought treatment on 5-20-19.

The injured employee and another employee were moving a large piece of plate steel. The injured employee dropped their end and the plate steel struck their leg and landed on their foot.

The employee had just gotten finished changing screens on the JCI screen. As ee was rushing to get back to the MCC to get the plant started back up, ee missed the second from the top step and fell approximately 10' to the ground, landing on left foot. The employee complained of pain in ankle. No fracture or deformity observed. Became a loss work injury on 4/19/2018.

The employee was moving a metal beam from one area to another. There were lifting chains on the ground nearby that the employee didn't see. While carrying the beam, the employee stepped on the chains, the chains and the employees foot slid out from under EE, and EE's knee twisted. Employee referred to doctor for evaluation.

The miner was walking from EE's welding truck to the parts room when EE's foot slid in mud on concrete. When EE's foot slid, EE's knee twisted. The miner continued to work until 7/24/2017, when EE was sent to the doctor. EE received PT and medication. Injury became reportable on 7/24/2017.

The miner was assisting a welder with fabrication activities when slag entered glove cuff and burned three fingers. The miner declined treatment at the time. The following Monday (7-17-2017), the miner complained of pain and swelling and was sent to an urgent care for treatment. Did not become a lost time accident until 7-18-2017.

The employee was doing excessive shoveling the previous Wednesday, woke up Thursday with back a little sore, worked Thursday, woke up Friday with intense back pain, but still came to work. Injury reported 1/20/2017 and became a lost time accident 1/23/2017.
