EE working on primary pit crusher when somewhere in the line of equipment was struck by lightning which resulted in the current flowing through the wrench EE was using on the equipment. The electrical current exited EE's arm. EE was knocked onto the ground. Emergency services called and EE was taken to the ER.

EE was putting a piece of steel in the steel dumpster and it caught onto EE's glove, causing EE's right ring finger to get smashed between 2 pieces of steel.

EE fell off a trailer while unloading a screw. The accident was noted but no action taken at the time. In December ee went in for a hip replacement surgery that resulted from earlier fall that caused a broken femur.
EMPLOYEE WAS MANUALLY MOVING ROCK THE SIZE OF A BASKETBALL FROM ATOP A CRUSHER WITH GLOVED HANDS. WHILE PICKING UP THE ROCK TO THROW TOWARDS CRUSHER HOPPER, ROCK SHIFTED AND PINCHED LEFT INDEX FINGER BETWEEN ROCK AND HINGE. RESULTS WAS BROKEN LEFT FINGER TIP AND 5 STITCHES ON FINGER.

INJURED WAS TAKING BROKEN REDUCER OFF THE SHAFT.HE HAD THE FRONT COVER OFF AND HAD SPLIT THE BUSHING.INJURED THOUGHT HE COULD HANDLE THE OTHER HALF OF THE REDUCER WHEN HE PULLED IF OFF.IT CAME OFF HE REALIZED THAT THE WEIGHT WAS MORE THAN HE SHOULD HANDLE AND PUT IT DOWN.
EE ALLEDGES THAT OPERATING A LOADER FOR YEARS HAS CAUSED THE PAIN HE HAS IN HIS HAND WRIST & SHOULDER DOC HAS EE WEARING A WRIST BRACE & ON 06-01-92 DETERMINED THAT EE HAS BURSITIS
EE COMPLAINED ABOUT TINGLING IN HIS HANDS HE HAD BEEN TIGHTENING CRUSHER BOLTS THE DAY BEFORE BY SWINGING A SLEDGE HAMMER.
