EE & co-worker were lifting the bearing assembly into place. Not able to fit it, they were bringing back into the manlift when the flange caught on the top railing. Assembly came to rest on EE's right forearm pinching it between the top rail and assembly.

Was walking down steps from wash plant and rolled right ankle causing stain.
EE was traveling along road way to another plant. As he was approaching a hill he moved to right side of roadway. As he proceeded the check engine lights came on. He diverted his attention to the lights and front tire came in contact w/ ditch. Causing truck to tip.
EE was putting V-Belt onto sheaves as belt went onto pulley the belt pinched his Right thumb between the two.
EE claims lower back pain from 5-10-12 - 5-29-12 When he finally reported to supervisor states back became sore from turning in loader to look back and rough roads along with defective seat.

Employee was tightening the cap on the core using a 16 pound sledge hammer. it was getting dark and light conditions were low. he swung at cap with hammer missing it. He attempted to stop his fallen thru and strained his lower back.
Employee alleges strain to upper right back and shoulder area from turning around and looking behind him. Also states ground was rough and bumpy.
Operating dozer for past several months. Employee complains of upper back strain B/T shoulders.
Employee stated he strained his upper shoulder/back area climbing into screen plant to change screen Did not seek medical attention until 8/5/11. Claim was under investigation while employee continued to work.

THEY WERE PUTTING A STEEL PLATE IN THE BUCKET ON THE LOADER & EE GOT HIS FINGER BETWEEN THE PLATE & A BOLT ON THE CUTTING EDGE AND SMASHED THE TIP OF HIS FOREFINGER THE BONE ON THE END OF HIS FINER IS BROKEN.
