The miner was using a hand grinder to remove a burr from a piece of metal when a piece of debris bypassed his safety glasses striking him in his left eye.

Individual was changing a truck tire and tried kicking it loose to get if off of the hub. The kick resulted in lower back pain right side and right leg pain. The employee went to Doctor a week later and he gave him prescription(s) and he returned to work with restrictions. Is following up. Employee was instructed to use a hammer next time.

Employee was loading prills on the blasting truck. A bag shifted under his foot. He realized he was going to fall so he jumped from the truck bed to the ground. He hurt his right knee. Did not want to go to the doctor and keep on working. The next morning his knee was swollen but he still would not go the the doctor. He said it was all right.

Employee was holding a center sleeve which is attached by nails to the end of a roll of belting upright while another EE was hitting the belt with a hammer to free it from the sleeve. His left index finger was smashed by the hammer. He was taken to the ER & received stitches.

EMPLOYEE WAS INSPECTING CONVEYOR BELT FOR PROPER TRACKING, HE HAD A LARGE WRENCH IN HIS LEFT HAND, HE SAID HE TURNED TO CLIMB OUT FROM UNDER CONVEYOR & STUCK THE WRENCH INTO THE ROLLER. HIS L EFT ARM WAS PULLED BETWEN ROLELR AND BELT. HE WAS ABLE TO PULL HIS ARM FREE OF BELT. EMPLOYEES LEFT ARM, SHOULDER & CHEST MUSCLES ARE SPRAINED, ALSO 2 BROKEN RIBS.
