Employee was installing driveline on Caterpillar 769 Haul truck, driveline fell on outside lower right leg above ankle.
Employee had cut a rubber mat with a razor knife. While picking up the mat, EE struck left thumb with the open razor knife cutting EE's thumb.

Miner was looking for A/C leak when EE's foot slipped from the door way to the track approximately 2 feet. The miner refused medical treatment at the time of injury stating EE's ankle was just sore. The miner stated EE's ankle had not improved on 11/21/2018 and requested treatment, the MRI indicated EE's left Achilles tendon was torn.

The miner was using an acetylene torch to remove an unsupported section of an iron platform. The miner chose to make the final cut from below and to the side instead of from above the section. The section fell at an angle striking the miners right foot breaking the cuneiform bone.

Employee was climbing down ladder on 773D haul truck stepped on a 41/2" rock and twisted his right knee
IE was using a hand tool (wrench) to tighten a bolt on skirting rubber, when his hand slipped and got smashed between wrench and steel beam.
After refueling, stepping up his ladder into the loader and felt pain just behind and slightly underneath left knee.

IE was standing to side of a forklift when the operator of the forklift accidentally pushed on the gas instead of brake while IE was assisting in loading a part onto the forklift. Part of the forklift(Not the forks) struck the IE in the side, likely bruising him. Has not returned to work because of other findings by Dr.
Turned head to the right to see behind him and felt pain in neck, right shoulder, right arm.
IE's tip of left index finger was smashed during a repair on a hammer drill. After taken to the emergency room, at approx 3PM IE's finger was removed back to just below the finger nail. IE returned to work on next scheduled day 3/10/13.
Employee was hammering out a bolt in a hole when the hammer struck the washer that was still on the bolt, the washer hit the employee's lip cutting it open. Employee was wearing safety glasses. No lost time. 4 stitches were given.

Tail pulley guard had been removed on the oversize stacker so work on it could be performed. Employee was standing by the guard waiting for the skidsteer to move in so they could attach it and put the guard back on. A high gust of wind blew the guard over and into employee hitting him in the side.

Driving on socket to install bearing, socket broke and cut him by knuckle between pointer finger and middle finger, requiring 5 stitches.
