Employee alleges EE was changing bucket teeth on a loader and EE tried to tap the end of tooth with a hammer. A small piece of metal came off the tooth striking EE in the chest. EE had surgery on 09/14/2020 to remove the piece of metal.

EE ALLEGES HE WAS ADJUSTING A CONVEYOR BELT TAIL SECTION, THE BAR WAS CAUGHT IN TROUGH IDLER. THE BAR PULLED HIS HAND INTO THE ROLLER AND CAUGHT HIS RIGHT INDEX FINGER BETWEEN THE ROLLER AND B ELT.

EMPLOYEE WAS SORTING SCREENS, THEY WERE STACKED VERTICALLY AGAINST THE TRAILOR WALL. HE MOVED THE FIRST SCREEN AND THE REST OF THE STACK CAME DOWN AND PINNED HIS LEG AGAINST THE SCREENS ON THE OPPOSITE WALL.
