EE WAS CLIMBING LADDER ON LOADER TO ACCESS THE CAB AND HIS FOOT GOT CAUGHT ON THE STEP AND HE FELL AND CUT HIS RIGHT ELBOW WHICH REQUIRED 4 STITCHES. HE ALSO HURT HIS LEG.

DRIVER WAS DUMPING LOAD OF COAL INTO HOPPER' WHEN TRAILER WAS EXTENDED 1/2 WAY UP, TRACTOR/TRAILER TURN OVER ON ITS PASSENGER SIDE, FIRST ONTO ANI-BEAM & THEN FELL FLAT ONTO STEEL SURFACE OF R AMP. THIS WAS RESULT OF COAL FREEZING IN THE TRAILER ON THE PASSENGER SIDE. DRIVER ATTEMPTED TO JUMP FROM THE TRUCK & IN DOING SO FELL TO THE GROUND CAUSING INJURIES.
