Employee struck top of his head on left side mirror of Cat 950 loader as he was climbing up to the cab causing him to strain his neck. Employee jarred his neck resulting in a headache and pain from his neck to lower left hip.
Employee injured his back when he repositioned his body to clear a self closing gate at the top of a fixed ladder. The injury was a herniated disk in lower back (L4). Although the employee injured his back on 9/18/2012, the first lost day of work was 9/24/2012.

Victim & co-worker were cleaning a belt line w/ a jackhammer & vacuum truck. Oversized material plugged the 6" vacuum hose, victim grabbed and removed the oversize material with his left hand. Breaking the material free from the hose end, his left arm was sucked into the hose up to his shoulder. Co-worker immediately activated a valve to stop the vacuum freeing the victim.

Dr. visit: 1-20-09 Emp. was standing beside step ladder. 2nd emp. was on the ladder with a torch heating the solinoid trying to get the valve to open. The ram on the 40 pound valve broke off and the valve fell about 12 feet, struck #1 emp. in the left hip.

Open fracture to 3rd finger, left hand. Mill spout slipped on come-along and pinched finger between the spout and the mill.
Left knee strain when attempting to turn broken packhouse screw with a 48" pipe wrench. The screw coupling piece collapsed and the wrench slipped causing the worker to fall backwards onto his left knee. The sudden transfer of weight to the left knee caused the worker to fall, further straining his knee.

Employee was ascending Terex ladder access to cab. Boots were muddy, area had been recently watered down for dust control. At the transition point from the ladder to the cab platform, employee's left foot slipped off the ladder rung and he fell off the ladder to the ground (approximately 9 feet).

When the employee was relocating a ladder while doing repair work in the quarry crusher building, the ladder struck a chain which was hanging from a support. The chain started to swing and struck the employee in the mouth. Two teeth were chippped.
At approximately 4:40 p.m., supervisor noticed a smoldering burn hole in the flex coal pipe which connects the #2 kiln and burner pipe. He immediately used a fire extinguisher and extinguished the fire. The system was shut down and the pipe was replaced. System was restarted at 6:20 p.m.

On 9-13-2005, employee came into the office and explained he had been kneeling and using his hands to put the pleated cartridges in the #3 baghouse and felt pain in his lower back. Employee explained that he hadn't been doing anything unusual. On 9-10-2005, employee stated his back was more painful. Arrangements were made for him to see a doctor.

Employee was drilling a hole in a piece of metal, the drill was winding down and the glove on his left hand got caught in the drill bit. The sudden motion twisted his hand, and his knuckle struck the metal surface. Employee continued to work for two days and on the third day felt he should have it looked at because of the swelling and pain his knuckle.

EE has had previous back problems and is currently off work (effective 11/5/04) and is preparing for back surgery. His physician has related all current problems to the Aug 2002 incident.
Employee was changing out a heater core, working under the dash. His right hand bumped the piece of metal, re-opening a non-occupational wound which required 8 stitches to close.

Repairman changing the liner in the hammermill. The worn plate was being lifted out of the hammermill and started to fall, he reached over the side of the hammermill to stop/control it. His right index finger went into the worn spot and was caught between the liner and the edge of the hammermill, cutting the tip of his finger off.

EMPLOYEE WAS MOVING A MOTOR WHICH WAS IN A CRATE. HE WAS MOVING IT BY PULLING ON THE CRATE. THE TOP BOARD OF THE CRATE BROKE OFF AND HE FELL BACKWARDS ONTO ANOTHER MOTOR. HE COMPLAINED OF A SO RE FINGER AND WRIST AND THE X-RAY PROVED HE BROKE HIS DISTAL METACARPAL IN HIS WRIST.

THE ACCIDENT OCCURRED AT THE START OF THE SHIFT WHEN AN EMPLOYEE WAS HOISTED IN A CAGE BY A HYDRAULIC CRANE TO CHANGE A YARD LIGHT ATOP A 30 FT. POLE. AS THE CAGE WAS HOISTED, THE HOOK BLO CK WAS BROUGHT VERY CLOSE TO THE SHEAVE ON THE END OF THE BOOM. IN AN EFFORT TO BETTER POSITION THE CAGE, THE BOOM WAS EXTENDED WITHOUT ALLOWING ANY SLACK IN THE CABLE CAUSING THE HOOK BLOCK
