Employee was climbing the stairs on the preheat tower and called over the radio to a co-worker that they needed to rest and felt dizzy. On 3/15/17, employee tripped, fell on right knee, and experienced minor knee swelling. EE was recovering as expected until EE started performing a lot of maintenance work on EE's knees. On 3/31/17 EE's conditioned regressed and EE sought treatment from EE's PCP. X-rays revealed no broken bones; diagnosed with deep tissue bruise on knee cap and placed on restricted duty.

The elevator contractor arrived on site, determined a 3 amp control fuse had blown, replaced the fuse, and the employee was released. Total time in elevator was 55 minutes.

Injured employee (IE) was assisting in the retraction of an inlet probe by means of a hand operated chain lever hoist. During the retraction process a ¼" eye and eye wire rope sling being used failed/broke and the rope snapped back and struck the IE's right wrist and IEE also lost balance and fell onto left buttocks resulting in wrist and coccygeal strain and contusion.

Injured employee was troubleshooting a baghouse tipping valve. After isolation and removing material (lime dust) EE moved in for a closer look through an approx. 1/2"H x 8"W opening in the valve. Unexpectedly, dust came out of the opening and hit the employee in the face resulting in dust getting into the employees left eye, medical treatment, and prescription medication.

Three days later went to the emergency room because of pain, was told they had a hernia, and the doctor said stay home for one day. IE is on a 10 pound lifting restriction.

Three contract office cleaning personnel contacted the control room operator and told EE the passenger elevator stopped working. The operator notified the shift supervisor. The shift supervisor told the operator to have the contract elevator company respond. They were in the passenger elevator for more than 30 minutes and MSHA was notified of the event.

Injured employee (IE) was trouble-shooting controls for an overhead hoist from a coal mill walkway. IE left the area and fell/stumbled forward while transitioning from one walkway to another. IE's right ear struck the mid-rail of an adjacent guardrail resulting in a 1cm laceration requiring two sutures. This is the first time the IE worked in the area.

The stand got hung up so he lifted it a little and it shifted slightly down instead of up, as expected, and pinched his right thumb tip between the stand and the screw trough cover. Result was a fractured thumb tip and nail avulsion.

Injured employee was cutting a zip-tie holding four half inch air hoses together. After positioning his utility knife blade between the zip-tie and hoses he pulled the knife upward. Apparently, his gloved left hand was closer to, or was pulled closer during the cutting and pulling action, the blade than expected because his left hand/thumb area was cut and required six sutures.

Once free, the pipe slipped from his left hand and the weight moved his right arm downward quickly. His right elbow struck the corner of a ladder resulting in a hyperextension and bicep tendon tear.

IE then sought treatment by his PCP and is awaiting a MRI.

Their actions were working; however, coordination failed once the probe starting moving out. The tip of E1's left pinky finger got smashed between the sledge and the flange causing a fracture and laceration requiring sutures.

DR determined that he had a 2nd degree burn from an interaction of high alkaline dust with his perspiration.

While trying to push a drag pin back into the link, a hoist was wrapped around the drag link and across the head of the pin. EE was operating the hoist applying pressure to the pin. While applying pressure, the pin moved suddenly, causing the handle of the hoist ot rotate, EE's hand impacted the side of drag housing. EE was placed on restricted duty on 5/29/09 for one day.

Seen in ER 5/23 & 5/25/09. 5/26 his work status was listed as totally disabled.

The EE claims the injury occurred on 12/15/08 but he didn't report it until 12/29/08. The EE was running exhaust hose for the TGA. He stepped up on the first step of a ladder, lost his balance and in doing so he put his hand out to catch himself and struck his hand on a table. EE was seen by doctor and x-rays were taken revealing a bone contusion to his R little finger.

After the driver had closed the hatch on his truck, he pulled the carbis platform up. The platform did not latch and began to go back down. As it did so the driver tried to catch it and reached across and grabbed a stair. As he pulled it up, his palm was pinched between the stair and the handle, causing the driver to need ten stitches.
